Objective: To investigate the factors influencing cesarean rates and explore strategies reducing cesarean rates.Method:1. 544 parturients at the city'smaternity hospital were selected from January 2013 to December 2013. Research was performed in the form of the questionnaire at the end of 42 days after delivery, all parturients truly complete questionnaire by themselves.2. Questionnaire includes the following items:( 1) parturients' age, occupation, education level, household income, birth history, as well as the trend of the choice of delivery in early pregnancy, and the awareness of cesarean and natural delivery;( 2) Prenatal care for parturients during pregnancy, prenatal learning, maternal activities and medical records during pregnancy, as well as family, friends and colleagues' suggestions and the mode of delivery parturients eventually want to choose.( 3) Situations of delivery including the mode of delivery, maternal and newborn health, as well as the late recovery and satisfaction.3.Statistical Methods : To establish a database using Epidata3.02, data were analyzed using SPSS17.0 statistical software, data(count data) were firstly analyzed by univariate analysis using Pearson chi-square and paired chi-square test, multivariate analysis was performed using logistic regression analysis method.Result:1. Univariate analysis explores factors affecting the rate of cesarean :(1) According to maternal demographic data, there is a significant correlation between age and the mode of delivery(P <0.05),and palace production is more suitable for women over the age of 35. Whereas education for parturients, average household income and parturients self-evaluated healthy conditions had no effect on choice of mode of delivery(P> 0.05);(2) In terms of understanding of cesarean, the majority(56.1%)of parturients who select cesarean think cesarean is less painful, 94.8% parturients who didn't select cesarean think natural delivery is beneficial for child, 93.7% parturients think natural delivery is favorable for quicker recovery. In addition, parturients' awareness affect the mode of delivery;(3) Suggestions from the family, husband, friends, and doctors will have an impact on the mode of production;(4) Pregnancy sports and attending pregnancy school have impact on the mode of delivery, whereas the number of check-ups has no impact on the choice of mode of delivery;(5), Ward level, the environment and other parturients' choice during hospitalization have impact on the choice of mode of delivery, while hospitalization and care of the doctors and nurses have no effect on the mode of delivery.2.Multivariate analysis explores factors affecting the rate of cesarean: Multivariate analysis demonstrated that age, chosen mode of delivery in early pregnancy, prenatal doctor's advice and other parturients' choice affect the final choice of the mode of delivery. Conclusion: To sum up, the choice of delivery mode was affected by many factors. age, awareness of cesarean, medical workers' advice and surrounding environment will affect the choice of delivery mode. To reduce the rate of cesarean, reinforcing propaganda and education,improving environment of the hospital, and increasing the number of hospital beds should be performed in hospitals; reducing the cost of cesarean,reforming income system of obstetricians strengthening research on painless childbirth and formulating in detail the plan should be charged by government. Meanwhile, parturients should adjust the mood, release the pressure on natural childbirth,build their confidence and be accompanied with friends if necessary to reduce cesarean rate.
