Objective:Researching the effect of China's current type 2 diabetes in community-based intensive health education intervention, and to explore the main influencing factors on the effect of community-based intensive health education intervention, and to give advice.Methods:1.Meta analysis of the effect of community-based intensive health education intervention in type 2 diabetes.Through Chinese CNKI, Wanfang Data,VIP journals full text database,from January 2010 to December 2015. According to the inclusion criterias and exclusion criterias, collection of relevant research literature. The combined RR value of blood glucose control rate is the effect size. Meta analysis was used to study the effect of community-based intensive health education intervention on blood glucose control in patients with type 2 diabetes mellitus. Meta analysis of subgroups were including comprehensive intervention methods for blood glucose monitoring and intervening time. 2.The study on the strengthen health education intervention in community-based patients with type 2 diabetes mellitus.Selecting a community health service center, Cangshan District, Fuzhou City in type 2 diabetic patients(diagnosis by hospitals; disease duration of at least 1 years;simple or complicated with hypertension). Research object has 60.Studying the effect of strengthen the follow-up health education in community-based patients with type 2diabetes mellitus, and taking self before and after research study. Intervention time for3 months, the subjects of intervention is medical college volunteers, the form of intervention is packet intensive follow-up intervention, intervention program mainly includes two diabetes of health education lectures, on diet control, exercise health and medication guidance, regular blood glucose monitoring, foot care and so on.According to patient requirements, once every two weeks of intensive follow-up intervention.Before and after intervention, the levels of glycosylated hemoglobin were detected, and the situation of awareness and behavior improvement were investigated. After finishing, spss 20.0 software was used for the statistical analysis, the selected paired test intervention were compared before and after glucose control rate, awareness rate,improvement rate effect; single factor analysis by two independent sample test and exact method; multi factor analysis using binary logistic regression analysis.Result: 1.Meta analysis of the effect of community-based intensive health education intervention in type 2 diabetes.(1)There were community-based intensive intervention in the fasting blood glucose control(I2=83.4%, combined RR value=1.37,95%CI(1.26~1.50), P<0.001). There were community-based intensive intervention in the postprandial 2h hours(I2=54.6%,combined RR value=1.16,95%CI(1.07~1.25),P<0.001).There were community-based intensive intervention in the Hb A1c(I2=22.2%,combined RR value=1.50, 95%CI(1.35~1.67),P<0.001).(2)Comprehensive intervention method for blood glucose monitoring(I2=46.1%,combined RR value =1.14, 95%CI(1.06~1.23), P=0.001),comprehensive intervention method for non blood glucose monitoring(I2= 85.6%,combined RR value= 1.28,95%CI(0.86~1.91), P= 0.230).There were significant differences between the intervention group and the control group in the comprehensive intervention mode of blood glucose monitoring and the non blood glucose monitoring method.(3)According to the subgroup analysis of intervention time, the difference of the community strengthened comprehensive intervention group and the control group were statistically significant.2.The study on the strengthen health education intervention in community-based patients with type 2 diabetes mellitus.(1)There is a significant difference in blood glucose control before and after the intervention(P<0.001).The difference has statistically significant in blood glucose control rate(P<0.001).(2) The difference is statistically significant in the knowledge about prevention and treatment of diabetes.(3)The rate of improvement in the behavior of patients with diabetes increased from33.33% to 77.19%, there is statistical difference(P<0.001). There is non significant differences in the behavior improvement of diabetic patients in the durg(P=0.210).(4)The single factor analysis show that, before the intervention, the behavior and the age of the intervention are the main factors the effect of strengthen health education in community-based patients with type 2 diabetes mellitus; univariate and multivariate logistic regression analysis shows that, awareness of the outcome of the intervention is the main factors the effect of strengthen health education in community-based patients with type 2 diabetes mellitus.Conclusion: To strengthen health education intervention on fasting blood glucose,postprandial 2h blood glucose, glycosylated hemoglobin control effect is clear.Community-based intensive health education intervention is conducive to improving the blood glucose control rate, awareness rate and behavior improvement rate inpatients with type 2 diabetes.
