Investigation of questionnaires of community elderly type Ⅱ diabetic patients consists of three parts. The first part is the questionnaire for diabetic patients’health condition. The second part is the questionnaire for diabetes knowledge, using the "Diabetes Knowledge Questionnaire4" provided by China Diabetes Education Project Team of Project HOPE. The third part is the questionnaire for self-management behavior of diabetic patients to make correct and appropriate assessment of the diabetic patients. According to the assessment of patients’ self-management condition and cognition degree of diabetes knowledge, individualized diet and exercise program are developed, targeted psychological care is implemented, and one-on-one health guidance is made, including basic knowledge of diabetes. From the third week, nurses organize diabetic patients to take part in various activities by exchanging thinking, attending lectures, watching audio-visual slides. In the sixth week, the questionnaires are issued again to assess the patients’ self-management and mastery of diabetes knowledge, and the changes are contrasted after implementation. Furthermore, investigation and evaluation of student nurses’ clinical communication skills are made by Nurses’Clinical Communication Skill Evaluation Scale. Result:This survey investigates a total of180people, of which18patients’materials are invalid due to various reasons. So the final sample are162patients.345questionnaires are distributed,338questionnaires are recovered,334questionnaires are valid, and the efficient rate is98.8%. The mastery of the disease knowledge and self-management skills and communication skills are increased significantly (P<0.01), the importance of dietary treatment and excises of diabetes are known, the do’s and don’ts of the medical treatment are mastered. By health education, diabetics’self-protection awareness and self-care awareness obviously are strengthened (P<0.01), and the patients’ blood sugar falls obviously (P<0.01). Patients are able to use what they have learned to implement self monitor, self care, balance food and reasonable dietary intake, actively taken part in their own activities, and mastered how to prevent against abnormal glucose levels. Among them, the reasonable control of dietary changes is the most obvious.Student nurses’communication skills are obviously improved after joining in the community diabetes health education (P<0.01). Diabetes Patients with different level of education usually have different knowledge level of diabetes mellitus. The higher level of education the higher knowledge level of diabetes mellitus. As a result, the effect of health education will be obvious. Those differences are of significance for statistics.(P<0.01). Diabetes patients with different time of the disease usually have different knowledge level of diabetes mellitus. The shorter time of diabetes mellitus the less knowledge of it. However, the effect of health education are obvious. Those differences are of significance for statistics (P<0.01). Before the healthy education, only8.02%diabetes patients got knowledge of the disease in the two communities. After the education,68.52%diabetes patients got knowledge of the disease from education, which obviously adds the ways to command the knowledge of the diabetes.
