Objective : By observing the dynamic hemoglobin level of perioperative in intertrochanteric fracture patients;calculating total blood loss,dominat blood loss and hidden blood loss,to observe the association between blood loss and bone mineral density,and then analyze the reasons.Method: This study is to select 118 intertrochanteric fracture patients from June 2013 to June 2015 in Hebei University Hospital,record general conditions,ete age,gender,body mass index,fracture types.By observe the changes of patients' dynamic hemoglobin level,to calculate total blood loss, dominant blood loss and hidden blood loss,to evaluate the difference of hidden blood loss between the patients who have different bone density.Result:This study included 238 patients with intertrochanteric fractures,and there are118 cases met the inclusion criteria.According to Evans classification,there are Ic type 45 cases,Id type 60 cases,? type 13 case.There are 21 normal bone density patients,40 low bone density patients,57 osteoporosis patients.The total blood loss of patients,who have normal bone density,is 490.90±47.50 ml,the dominat blood loss is 91.14±8.34 ml,the hidden blood loss is 402.76±46.44 ml. The total blood loss of patients,who have low bone density,is553.98±53.26 ml,the dominat blood loss is 89.75±6.89 ml,the hidden blood loss is464.22±54.68 ml.The total blood loss of patients,who have osteoporosis,is 611.37±42.50 ml,the dominat blood loss is 88.98±9.15 ml,the hidden blood loss is 522.39±42.11 ml.The dynamic hemoglobin level of the patients is the lowest in the 3 day.The hidden blood loss with normal bone mineral density is lower than patients with low bone mineral density and osteoporosis,the difference is statistically significant(P<0.05).But the differenc of dominat blood loss is statistically insignificant(P>0.05)Conclusion:There are much hidden blood loss in intertrochanteric fracture patients.It is very important to supplement the blood volume during periopretive.
