Application Of A Self-made Guide Device In Minimally Invasive PFNA To Reduce Hidden Blood Loss In Elderly Patients With Intertrochanteric Fracture

Purpose Objective to study the application of self-made PFNA guider in proximal femoral nail antirotation(PFNA)internal fixation in elderly patients,and to explore its influence on surgical incision,operation time,postoperative dominant blood loss and postoperative hidden blood loss,so as to provide some theoretical support for the clinical application of self-made PFNA guider.methodsAccording to the exclusion and inclusion criteria,60 patients with femoral intertrochanteric fracture who were willing to accept surgical treatment in the Department of orthopedics,Wuhu Hospital of traditional Chinese medicine from January 1,2019 to December 31,2020 were selected and divided into two groups according to the principle of voluntariness.Group 1 was the experimental group.The patients in the experimental group were treated with self-made PFNA Guide(self-made PFNA guide minimally invasive PFNA group),and group 2 was the control group PFNA guider(routine PFNA group)was used.The operation time,incision length,blood loss during operation,gauze weight gain during dressing change,hemoglobin and hematocrit 2 hours before operation,the first day,the third day and the seventh day after operation were recorded,and the hidden blood loss was calculated by gross equation.Spss24.0 statistical software was used for statistical analysis.The operation time,incision length,total postoperative blood loss,apparent postoperative blood loss and hidden postoperative blood loss were compared between the two groups.Results(1)There was no significant difference in general information between the two groups(gender,age,height,weight,BMI,injury site,fracture classification,etc.)(P > 0.05).(2)There were significant differences in total blood loss,dominant blood loss and hidden blood loss between the two groups(P < 0.05),and the experimental group was better than the control group.There were significant differences in operation time and incision size between the two groups(P < 0.05),and the experimental group was better than the control group.ConclusionFrom the analysis of the results of the study,it can be concluded that the self-made PFNA guide can reduce the incision of PFNA operation,reduce the operation time,reduce the soft tissue dissection,and effectively reduce the hidden and dominant blood loss.
