The Application Of Crown Lengthening Surgery For The Residual Root Preservation And Restoration

Objective: Through the observation of clinical repairing effect with crown lengthening surgery for the residual root,in order to providing reasonable repairing treatment for preservation the root,and improving the saving rate of the root.Methods: To Selecting 30 patients with the residual root and dentition defect from June2014 to October 2015.There were 32 teeth with the residual root(anterior 10,premolar8,molar 14).The male is 18,20 teeth;the female is 12,12 teeth;from 22 to 64 years old,and the average age is 43 years old.The 32 teeth had through the root canal treatment for 1~2 weeks,the patient has no self-conscious symptom,without percussion pain.To applying the crown lengthening surgery after the local anesthesia,and beginning to post crown?connect crown ? fixed bridge?over denture?attachment dentures repairing after six weeks.Follow-up for 3~16 months,using success,poor,fail to judgment repairing effect.Success: patients have no self-conscious symptom,restoration margin closed,good retention,healthy gums,transparent coordinated with adjacent teeth,have no periodontal pocket ?loose teeth or alveolar bone absorption.Poor: gum color is normal,good restoration retainer,margin slightly exposed,alveolar bone absorption slightly.Failure: swollen gums or retreat,restoration edge exposure,restoration loose or fall off,alveolar bone absorption significantly.Results: The restoration treatment of 29 teeth with the residual root was success,without self-conscious symptom,restoration margin closed,good retention,healthy gums,transparent coordinated with adjacent teeth,have no periodontal pocket,have no loose teeth or alveolar bone absorption,the success rate is 90.63%.A incisor was poor,appearing "black triangle",but the beautiful effect was better than before after remaking.The rest 2 teeth were failure.The one was loose and shedding after 12 months,because of the occlusal trauma.The root loose I °,X-ray was alveolar bone absorption.The other appeared gum edge swelling and hyperplasia in the direction of dental crown more than 1 mm,X-ray showed periodontal membrane gap had widened.Conclusion: If you can master the indications,the crown lengthening surgery can be used to save the residual root and to prolong the life of tooth,recovering beautiful and chewing function,avoiding alveolar bone absorption.
