A beautiful smile is an important sign of beauty and health. Many factors determine the overall beauty of a smile. However, within the aesthetic category determined by the smile, there is a group of patients who show a smile with gingiva, uneven gingival margin and the crown is too short, etc.,which have different degrees of influence on the beauty of a smile. With the rapid development of economy, people's requirements on the aesthetic appearance of smile are increasingly improved. In order to solve the above phenomenon, crown lengthening in the maxillary anterior region has attracted more and more attention.Surgical crown lengthening?SCL?[1]adjusts the position and shape of periodontal soft and hard tissues by surgical methods,so as to extend the clinical crown, expose more healthy tooth tissues, restore the biological width, and achieve the effect of periodontal health.Due to the high aesthetic requirements of the anterior region, the traditional crown extension procedure can not achieve the desired effect because it cannot accurately remove the soft and hard tissues. In the aesthetic area of maxillary anterior teeth, the ideal crown lengthening should achieve a comfortable, minimally invasive, efficient and rapid improvement of the contour of soft and hard tissues,so as to achieve the periodontal aesthetic treatment. At present, minimally invasive and quantitative periodontal aesthetic diagnosis and treatment has been paid more and more attention by clinicians. This article demonstrates the surgical procedures and methods of minimally invasive crown prolongation in the anterior dentition area under the high requirements of periodontal aesthetics. Objective:To evaluate the aesthetic effects of minimally invasive crown lengthening surgery on gummy smile patients. Methods:We implemented a prospective observation for patients who suffered from gummy smile and of course underwent a minimally invasive crown lengthening surgery. We use plaque index?PLI?and the mobility?MOB?to evaluate the gingival and periodontal conditions. The aesthetic effects of the treated teeth were evaluated with the smile line level.Visual analogue scale?VAS?was used to examine patient's satisfaction degree before and after treatment. Results:One week after minimally invasive crown lengthening, the PLI increased, but after oral hygiene instruction,PLI decreased at 8 weeks and 6 months after surgery, respectively.MOB had a growing trend at 1 week after surgery and stabilized at 8 weeks after operation. The patients were satisfied with the gingival aesthetic effect of the maxillary anterior teeth, with the patient's smile line reaching the class?and class?standards. Conclusions:Patients could attain satisfactory clinical effects using the minimally invasive crown lengthening surgery, combined with the correct selection of indications, delicate operation and regular oral health care.
