Objective: To analyze the characteristics of neuropsychological cognitive impairment in PD patients with different stages and provide a reliable basis for the establishment of clinical sensitive,simple cognitive impairment in PD survey instruments.Methods: From December 2014 to December 2015 in our hospital neurology clinic,50 PD patients and 50 healthy volunteers was selected as a control group.Complete the mini-mental state examination(MMSE)in the resting state,the Montreal Cognitive Assessment(Mo CA),verbal fluency test(VFT),Wisconsin Card Sorting Test(WCST),Benton line direction determination,paced auditory continuous addition(PASAT),digit symbol coding test(DSCT),the California language learning test(CVLT)and other surveys and analyzed.Applied Simultaneously Hoehn-Yahr staging of the severity of PD patients were assessed to analyze the characteristics of cognitive impairment in PD patients in different periods.Results: 50 patients with primary PD patients,31 cases of early group,23 males and 8 females,mean age(55.21 ± 11.39)years,education time(4.76 ± 9.33)years;advanced group of 19 patients,male 13 Example 6 females,mean age(61.88 ± 20.19)years,education time(5.56 ± 8.91)years;control group of 50 patients,including 33 males and 17 females,mean age(60.13 ± 11.32)years,subject to Education time(6.38 ± 3.85),the two groups of gender,age,education,the difference was not statistically significant(P> 0.05).In the main overall evaluation of cognitive function MMES scale in early PD group than in the control group,but the difference was not statistically significant(P> 0.05);advanced PD group compared with the control group difference was statistically significant(P <0.05);group of early PD and advanced PD group decreased at the same time,but the difference was not statistically significant.In Mo CA scale measurement,early PD group than the control group,but the difference was not statistically significant(P> 0.05),while the control group and advanced PD early PD group scored significantly decreased compared with the differences were statistically significant(P <0.05);the number of correct answers and continuous evaluation of the number of errors in early PD group compared with the control group scored lower,the difference was statistically significant(P <0.05);but it is rather the number of classification measurement is completed,the difference was not statistically significance(P> 0.05).Advanced PD and control groups in the number of correct answers,continuing errors and number of categories completed assay were statistically significant(P <0.05).Early and late PD PD group compared with the group,the three were not statistically significant(P> 0.05).Memory test results show that early PD group compared with the control group,short-term memory and delayed memory scores compared with no significant difference(P> 0.05);and advanced PD group compared with early PD and control groups,short when memory and delayed memory score was significantly decreased,the difference was statistically significant(P <0.05).Verbal fluency test results show that early PD compared with the control group,the difference was not statistically significant(P> 0.05);and advanced PD group compared with early PD and control groups,the number of Chinese words and the number of animals named scores were significantly decrease compared with the two,the differences were statistically significant(P <0.05).Attention and information about the test results show that compared with the control group,early PD patients PASAT,DSCT score lower,the difference was statistically significant(P <0.05);advanced PD patients PASAT,DSCT score also decreased significantly,the difference was statistically significance(P <0.01);compared with patients with advanced PD patients with early PD group,PASAT and DSCT scores were significantly decreased,the difference was statistically significant(P <0.05).Conclusion: The cognitive function in patients with early PD damaged mainly in executive function,attention,information processing speed;and advanced PD patients may exist a wide range of cognitive impairment,including executive function,information processing speed,attention,memory,language fluency were significantly damaged.PD patients by selecting sensitive and appropriate rating scale can be found early and accurate cognitive dysfunction in PD patients,and for different areas of cognitive impairment,corresponding psychosocial rehabilitation therapy and cognitive impairment may delay or prevent dementia.6 Objective: To investigate the changes of serum uric acid functional understanding of the impact on the different stages of Parkinson's patients.Methods: December 2014 to December 2015 in our hospital neurology clinic 70 PD patients for the study group,also select 50 healthy volunteers as a control group.Applications Hoehn-Yahr staging of the severity of PD patients were evaluated,Mo CA Scale PD patients with cognitive impairment assessment,based on clinical data were statistically analyzed,and fasting serum uric acid and creatinine clearance,comparative analysis PD prevalence of cognitive function and disease classification and serum uric acid levels if there is a correlation.Results: 70 cases of PD patients,41 males and 28 females;aged 48-88 years,mean 67.3 ± 14.61 years,duration 9-189 months,mean 81.5 months.Wherein the initial diagnosis of PD 48 cases.Application HY staging the severity of PD motor symptoms assessed,of which 40 cases of early disease and 30 cases of advanced disease;press Mo CA scores were divided into 45 cases of non-cognitive function disorder group,25 cases of cognitive impairment group;50 healthy volunteers,30 males and 20 females,aged 41-79 years,mean 61.6 years old.Early PD group,advanced PD group and control group in age,sex,body weight difference was not statistically significant.Early PD compared to the control group,blood uric acid decreased,the difference was statistically significant(P <0.01),endogenous creatinine clearance rate of early PD compared with the control group no significant difference(P> 0.05);advanced PD control group compared to the blood uric acid decreased,the difference was statistically significant(P <0.01),endogenous creatinine clearance rate of early PD compared with the control group no significant difference(P> 0.05);early PD uric acid drops below late PD,but the difference was not statistically significant(P> 0.05);compare the three,blood uric acid decreased significantly(P <0.01),but the endogenous creatinine clearance rate was no significant difference(P> 0.05);no cognitive impairment group with cognitive impairment group fell by an average of hematuria acidic water,but there is cognitive impairment group uric acid levels decreased significantly lower than those without cognitive impairment group,the difference was statistically significant(P <0.01);and endogenous creatinine clearance rate between the two groups showed no significant difference(P> 0.05)Conclusion: first,blood uric acid levels in patients with different stages of PD decreased,but the difference was not statistically significant,can not be used to assess disease severity levels of uric acid;second,the severity of cognitive function in PD patients with different levels of uric acid and cognitive function related to worse cognitive function who uric acid levels decreased more;thirdly,serum creatinine clearance rate did not change significantly in PD patients.
