Clinical Comparative Observation Of Shoulder Swing Therapy Combined With Splint Fixation And Surgical Treatment For Neer Type Ⅲ Humeral Surgical Neck Fracture

Objective:Comparison of therapeutic effects of shoulder joint swing and splint fixation and proximal humeral locking plate fixation for the treatment of humeral surgical neck fractures treated with Neer III.Through the analysis of the objective evaluation and statistics on the shoulder joint swing and splint fixation for the treatment of humerus surgical neck fracture clinical curative effect,evaluate the clinical efficacy and safety of this therapy.The therapy can be reliable and feasible in clinical application,and targeted.Methods:Methods to study to 2014-09-01—2015-10-31 Jiangsu Province Hospital of traditional Chinese Medicine Department of Orthopaedics and traumatology from 40 patients with Neer III of humerus surgical neck fracture patients.20 cases were treated with locking proximal humeral plate fixation(operation group),and 20 cases were treated with swing shoulder and splint fixation.The length of stay,fracture healing time and Constant-Murley score were compared between the two groups.Result:There were no statistically significant differences in gender,age and fracture pattern between the two groups(P>0.05).The length of hospital stay in the swing shoulder and splint fixation group was(13.96±1.20)days,it is shorter than the LPHP group with(14.65±2.37)days.and the bone union time in the operation group was(15.71±1.05)weeks,while the swing shoulder and splint fixation group was(13.96±1.20)weeks,there were significant statistical significance(P<0.01).The pain score,ADL,ROM,muscle strength,local shape and total score of Constant-Murley were not significantly different between the two groups after 6 months of operation(P>0.05).Conclusion:There was no significant difference in clinical efficacy between proximal humeral locking plate fixation and shoulder joint swing and splint fixation.But left shoulder is combined with splint fixation in the treatment group in fracture healing time and the duration of hospitalization was significantly higher than that of operation group of short,simple operation,low cost,as far as possible to reduce the secondary injury of soft tissue of shoulder joint and avoid caused by surgical trauma aggravates and destruction of blood supply,thus enabling to promote soft tissue function recovery and fracture healing.
