Clinical Observation Of Guipi Decoction Treating Cytopenias(Syndrome Of Dual Deficiency Of The Heart And Spleen) Of Hepatitis C Using Peginterferon Antiviral

Objective Through the observation of the patient's clinical symptoms, laboratory indexes and the adverse reaction occurred during the treatment, to evaluate the efficacy and safety of Guipi Decoction treating cytopenias(syndrome of dual deficiency of the heart and spleen) of patients with chronic hepatitis C or hepatitis C cirrhosis compensatory stage using peginterferon antiviral. It is intended to provide a basis for Guipi Decoction to treat the disease.Methods The research on the patients with chronic hepatitis C or hepatitis C cirrhosis compensatory stage using peginterferon antiviral and appearing cytopenias, TCM syndrome Differentiation for syndrome of dual deficiency of the heart and spleen. All the cases were derived from Hubei Provincial Traditional Chinese Medical Hospital outpatient and inpatient department at June 2014-2015 year in December. A total of 52 cases, according to the randomized design, 26 cases were divided into treatment group, 26 cases in the control group. All the patients were consistent with the indications of interferon combined with Ribavirin. Allpatients were given Peg IFN-?2a combined with Ribavirin.In addition, the treatment group was given Guipi Decoction, the control group was given Leucogen tablets, the treatment lasted for 12 weeks. Mainly observing the changes of blood analysis and TCM syndrome integral curative in two groups of patients before and after treatment to analysis Guipi Decoction's effect. In order to evaluate the safety, we observed the adverse events occurred and the changes of objective indicators, such as urine routine, liver and kidney function, blood glucose, thyroid function, electrocardiogram, blood pressure.Results :(1)Comparison of blood routine in the two groups of patients: compared with neutrophils and hemoglobin in the test group before and after treatment, the difference was statistically significant(P < 0.05), it indicates that Guipi Decoction can improve the neutrophils count and hemoglobin concentration; After treatment, neutrophils of the control group compared with those before treatment, the difference was statistically significant(P < 0.05) and prompt Leucogen can increase the number of neutrophils; After treatment, compared the neutrophils of test group with the control group, there were significant difference between two groups(P < 0.05), suggesting that Guipi Decoction increasing neutrophil is more effective than Leucogen. During the treatment, compairing the maximum decrease value of NEU,PLT, HGB in two groups of patients throughout the course of treatment, the difference was statistically significant(P < 0.05) and prompt Guipi Decoction can relieve the decline trend of platelet.(2)TCM syndrome Differntiation:Compared TCM syndrome totalscore of two groups of patients before treatment, the difference was not statistically significant(P > 0.05); After treatment, the TCM syndrome total score of the control group has no statistical significance(P > 0.05), while the test group,the difference was statistically significant(P < 0.05), This suggests that Guipi Decoction has efficacy in ameliorating the symptoms of patients. The symptoms scores of fatigue, anorexia, weight loss, insomnia, pale complexion minimalist and so on have no significant difference between the two groups(P > 0.05) before treatment. After treatment, the symptoms scores of fatigue, anorexia, insomnia, pale complexion minimalist, loose stools is reduced in the test group, the difference was statistically significant(P < 0.05); but the symptoms scores of emaciation, fervor, sweating, dizziness was not statistically significant(P > 0.05). This shows that Guipi Decoction can help patients relieve the symptoms of fatigue, anorexia, insomnia, pale complexion minimalist, loose stools.(3)The negative conversion rate of HCV RNA: After 12 weeks of treatment, the test group is 80.77% and the control group is 76.92%, there were no significant difference between two groups(P > 0.05), Guipi Decoction can not promote HCV RNA negative conversion.(4)In the observation, the test group patients have no adverse reactions after taking the decoction, and have no abnormal in urine routine, renal function, blood glucose. There were no liver function changes caused by the decoction. It shows that Guipi Decoction is safe.Conclusion : Guipi Decoction has better effect on improving neutrophil than Leucogen. Guipi Decoction can improve the decreasing degree of platelet,mitigating the anemia status, and relieving the symptoms of fatigue, anorexia, insomnia, loose stools,which shows that Guipi Decoction treat cytopenias(syndrome of dual deficiency of the heart and spleen) of patients with chronic hepatitis C or hepatitis C cirrhosis compensatory stage using peginterferon antiviral is efficacy significantly. During the treatment, we did not find hematopoietic system damage or liver and kidney function damage occurs with the patient after taking Guipi Decoction, the safety is worth affirmation. Guipi Decoction did not increase the negative conversion rate of HCV RNA, but it can improve laboratory parameters and some clinical symptoms of patients, it also has no significant side effects, which can improve the compliance of patients with antiviral therapy, so as to achieve the purpose of treatment.
