BackgroundWith more and more surgical methods to correct high myopia,FS LASIK surgery is still the most popular way.Meanwhile,the safety and efficacy of ICL in correcting high myopia has also been confirmed,this operation mode also becomes more and more popular.For the two groups of operation can correct the high myopia,it has great clinical significance in the refraction to carry comprehensive comparison of these two methods and choose the most suitable operation method.ObjectComparative study on the visual quality of FS LASIK and ICL V4 c group patients from both subjective and objective aspects.Method1.This study selected 80 patients with high myopia(a total of 80 eyes),FS LASIK group: 20 patients,with a total of 40 eyes.ICL V4 c group: 20 patients,with a total of 40 eyes.Compare the two groups' subjective and objective visual quality.Myopia equivalent spherical mirror number-6.00D—10.0D.2.Operation method: the FS LASIK group use Swiss LDV femtosecond laser to produce corneal flap,and then use American VISX STAR S4 IR excimer laser to cut the corneal stroma.The ICL V4 c group's lens is new central pass Visian V4 c lens,and the intraocular lens is implemented in the rear.Respectively compare the visual quality of the patients at preoperation,1 week,1 month,3 months,6 months and 1 year postoperation.Instrument selection(1)Objective visual quality instrument: Wave Scan Wave Front(American VISX's Wave Scan Wave Front)and OQAS Objective optical qualit? y analysis system(OQAS?,Spanish Visiometrics SL);(2)Subjective visual quality instrument: contrast sensitibity CS(CSV-1000 E,American Vector Vision)3.statistical method: the statistical analysis is performed using SPSS 19.0.Set the wavefront aberration of patients with 4mm pupil diameter;Under 4mm pupil,the OQAS?measured the value of OVAS,SR,OSI and MTF before and after the operation.Conduct paired t test and rank test of the two groups,the difference of P<0.05 has statistical significance.Result1.Diopter:Naked visions: The naked visions within 1 years of the two groups is significantly higher than that before surgery.After 1 weeks,3 months,6 months,1 years,the naked vision of ICL V4 c is better than the FSLASIK group.The best corrected visual acuity: The best corrected visual acuity within 1 years of the two groups is significantly higher than that before surgery.After 6 months,1 year,there were statistical differences between the two groups,the best corrected visual acuity of ICL V4 c is better than the FSLASIK group.The dominant equivalent sphere: after 1 week,1 month,there were statistical differences between the two groups,the variance of FS LASIK group is greater than that of ICL V4 c group.Astigmatism: The difference between the two groups has statistical significant before and after the operation(P<0.05).Within 1 year after the surgery,the difference between the two groups was statistically significant,and ICL V4 c is better than that of FS LASIK group.2.Objective visual quality?Wavefront aberration: after 6 months of the operation,the two groups were significantly higher than the preoperative,and there was statistically significant difference between vertical clover of ICL V4 c group and the preoperative.The vertical coma,horizontal coma,and spherical aberration of ICL V4 c group have significant difference with the preoperative.The vertical clover,horizontal clover and vertical coma of FS LASIK group has no significant difference from the preoperative,while the spherical aberration has.?OQAS?:MTF cutoff rate: the FS LASIK group's MTF cutoff rate reduced within 1 month after the operation,and then gradually increased;while that of ICL V4 c group gradually increase after operation.SR: FS LASIK group's SR decreased in the first three months,and then was recovered consistent with the preoperative;ICL V4 c group's SR decreased in the first month,and then was recovered consistent with the preoperative.OSI: The two groups' OSI were improved in the first three months,and then was recovered consistent with the preoperative.OVs: FS LASIK group's OVs decreased in the first month,and then gradually increased;while the ICL V4 c group was improved than the preoperative.3.Contrast sensitivity: in the case of photopic vision,there was statistical difference between the contrast sensitivity and the preoperative of ICL V4 c group after 6 month at 3.0,6.0,12.0,and 18.0c/d,the postoperative is better than the preoperative.However,there was no significant difference after 6 months of FS LASIK group.In the case of dark vision,the postoperative is better than the preoperative.There was statistical difference between the contrast sensitivity and the preoperative of FS LASIK group after 6 month at 3.0,6.0,12.0,and 12.0c/d,which has been decreased compared to the preoperative.In the case of dark dazzling,there was statistical difference between the contrast sensitivity and the preoperative of ICL V4 c group after 6 month at 1.5,3.0,6.0,12.0,and 18.0c/d,the postoperative is better than the preoperative.While the FS LASIK has statistical difference between the contrast sensitivity and the preoperative after 6 month at 1.5,3.0,6.0,12.0,and 18.0c/d,which has been decreased compared to the preoperative.Conclusion1.FS LASIK and ICL V4 c implantation are both effective methods to correct high myopia,and have good postoperative visual quality.2.Compared to FS LASIK,ICL V4 c implantation is a preferred choice for high myopia because of less aberration,better subjective and objective visual quality.3.Wave Scan Wave Front,OQAS ?Objective optical quality analysis system,and contrast sensitivity instrument can be used to evaluate the visual quality of the patients.
