Objective:The purpose of this paper is to observe the clinical efficiency of the simple thoracolumbar compression fracture with the therapy of Zhanjin Huoxue Decoction combined with Natural traction method supplemented with turritus pad and to explore the mechanism about it.Method:80 cases of inpatients: with the simple thoracolumbar compression fracture were chosen as the object of the study.They were conformed to 《Chinese medicine industry standards of The people’s Republic of China ·The standard of the diagnosis and efficacy of Traditional Chinese Medicine·thoracolumbar Fracture》.All patients were divided into the controlgroup and the Zhanjin Huoxue Decoction combined with Natural traction method supplemented with turritus padgroup by the method of random number table,with 40 people in eachgroup.There were 18 males and 22 females ingroup A.The mean age(x ± s the same below)of them was 67.0 ± 5.1 years old;There were 14 males and 26 females ingroup B.The mean age ofgroup B was 66.0 ±4.8 years old.Thegeneral situation of all patients of the twogroups had no statistical difference.The patients ofgroup A weregiven Gukang capsule orally.The patients did "five point support function exercise " under the doctor′sguidance during the first week of admission;The patients ofgroup B were treated by Natural traction method supplemented with turritus and Zhanjin Huoxue Decoction orally.Zhanjin Huoxue Decoction contained eleven kinds of Chinese herbal medicine such as: Angelica 12g,Chuanxiong 10g,Achyranthes 9g,Fructus aurantii 15g,Jiang Huang 15g,notopterygium 15g,Daemonorops draco Bl 10g,white atractylodes rhizome 20g,Poria 15g,angelica 15g,musk 0.1g(blunt).According to the fracture stage,we made adjustments.For example,in order to eliminate swelling and relieve pain,hanfangji 20g,orange 9g,and rhubarb 15g were added to the decoction;In the middle of treatment Peach 10g,safflower 10g and Radix Paeoniae Rubra 15g were added to the decoction to increase the speed of blood circulation and resolve the congestion.In the end of the treatment period,we added Drynaria 15g,Dipsacus 20g and Rehmannia 15g to nourish liver and kidney.The patients also did "five point support function exercise " under the doctor′sguidance during the first week of admission.A course included 12 weeks.The observation indexes included the wedge compression degree which reacted the degree of vertebral body compression,the healing time which reacted the speed of the healing process(unit: week),Cobb angle which reacted the degree of scoliosis and the PRO scale which responsed the satisfaction degree of the patients according to curative effect evaluation at the end of the thoracolumbar compression fracture.We measured the wedge compression degree and Cobb angle before and after the treatment of the twogroups and recorded the healing time of fracture.We followed up all the patients of the twogroups at the end of the course and 12 weeks after the course and we collected the data of PRO.We analyzed the data of the twogroups statistically.Wedge compression degree,Cobb angle and PRO scale score were compared betweengroups and withingroups.Fracture healing time was compared withingroups.Results:First,there were no significant differences in Cobb angle,Wedge compression degree before the course of disease.The Cobb angle,Wedge compression degree ofgroup A had no obvious change before and after the treatment(P>0.05).Cobb angle,Wedge compression degree ofgroup B had statistical significance before and after the treatment.We compared the two kinds of index ingroup B.The results showed they had significant differences(P<0.05).Comparing all patients′s cobb angle and Wedge compression ingroup A andgroup B,they had statistical significance(P<0.05);Second,there were no significant differences in Wedge compression degree and no statistical significance of all the patients in the twogroups.Wedge compression degree ofgroup A had changes before and after the treatment.By comparison the Wedge compression degree ofgroup A within thegroup,it had statistical significance(P<0.05).Wedge compression degree ofgroup B had differences obviously before and after the treatment and it had statistical significance within thegroup(P<0.05).There was significant difference in Cobb angle between the twogroups after treatment,and it had statistical significance(P<0.05);Third,according to the healing time of fracture that the X-ray showed as the standard,the average time ofgroup A was longer thangroup B.And the number of the patients ofgroup B was significantly more than those ofgroup A.Two sets of data had significant difference(P<0.05).Fourth,we followed up the patients of the twogroups in the form of questionnaire survey respectively in 12 weeks,24 weeks andgot the scores of PRO in the percentage of thoracolumbar compression fracture curative effect evaluation.The scores of PRO of the twogroups had significant differences after 12 and 24 weeks with thegroups and between thegroups,it had statistical significance.(P<0.05).Conclusion:Firstly,In terms of the restoration of vertebral height restoration and the physiological curvature of the spine,the therapy of Zhanjin Huoxue Decoction combined with Natural traction method supplemented with turritus pad was superior to the conventional conservative treatment(group A)to treat simple thoracolumbar compression fracture from the perspective of image comparison.Secondly,In the aspect of the healing time of the vertebral fracture,the therapy of Zhanjin Huoxue Decoction combined with Natural traction method supplemented with turritus pad was significant shorter than the controlgroup(group A).Thirdly,Following up the patients of the twogroups,we found that the patients were more satisfied with Zhanjin Huoxue Decoction combined with Natural traction method supplemented with turritus pad than the conventional treatment of group A.
