[Objective] This study was designed to investigate and compare the clinical outcomes of elderly patients with simple thoracolumbar OVCF using over-extension traction device combined with kyphoplasty(PKP)and PKP alone.[Methods] 76 patients who met the criteria for the elderly patients with simple thoracolumbar OVCF admitted to our hospital from January 2018 to December 2018 were randomly divided into two groups and screened according to relevant criteria.Finally,64 cases were obtained,the treatment group(32 cases)and the control group(32 cases).The treatment group was treated with over-extension and traction device combined with PKP,and the control group was treated with PKP only.The two groups were compared before surgery,1 day,1 month,and 3 months after surgery.The compression rate of the diseased vertebra was used to compare the height recovery of the vertebral body.The Cobb's angle of the spinal deformity was used to compare the spinal deformity.The VAS score was used to compare the pain.In the case,the lumbar spine JOA score was used to compare lumbar spine function.The statistic of the last two sets of data was used to analyze the differences between different groups within and after treatment.[Results] 1.The data of the two groups of elderly patients with simple thoracolumbar OVCF were compared.There were no significant differences in preoperative gender,age,cervical compression rate,Cobb's angle,VAS and JOA scores(P >0.05).2.There was no significant difference in VAS score between the two groups P >0.05).There were significant differences in VAS scores between the two groups before and after operation(P <0.05).3.There was no significant difference in lumbar spine JOA score between the two groups(P >0.05).There were significant differences in lumbar spine JOA scores between the two groups(P <0.05).4.The compression rate and Cobb's angle of the patients at each stage were significantly smaller than those of the control group(P <0.05).There was no significant difference in the compression rate and Cobb's angle between the two groups(P >0.05).However,the Cobb's angle and the compression rate of the diseased vertebrae were significantly different between the preoperative and postoperative stages(P <0.05).[Conclusion] In the treatment of elderly patients with simple thoracolumbar OVCF,manual reduction with overextension traction device combined with PKP in vivo and in vitro has good effect in improving the recovery of vertebral height,reducing the compression rate of diseased vertebrae,restoring the physiological curvature of the spine and reducing kyphosis.
