Objective: Chronic obstructive pulmonary disease(Chronic obstructive pulmonary disease,COPD)patients with recurrent disease exacerbation,progressive decline in lung function,clinical treatment is not ideal,easy to produce low self-esteem,guilt,anxiety,depression and other psychological disorders.Extended care after discharge is an open form of education and extension of health care.For patients with moderate to severe COPD in terms of quality of life after discharge will be even more serious threat.To this end,this study selected 70 cases in Hebei Chest Hospital from December 2013 to October 2014 period were treated with chronic obstructive pulmonary disease in patients with severe as research subjects,randomized controlled trials analyzing,evaluating patients with moderate to severe COPD in an interview with the quality of life extended care after improvement,the analysis extended care for chronic obstructive pulmonary disease in patients with severe lung function,physical restoration,the health levels and other indicators in order to severe chronic obstructive pulmonary disease in the hospital after recovery from disease reference,create favorable conditions for the health of the rehabilitation of patients with the disease,but can also help to promote the continuation of nursing process in clinical medicine which,with great practical significance.Methods: Selection of Hebei Chest Hospital in December 2013 to October 2014 treated 70 patients with moderate to severe COPD patients as research subjects,according to the patient's date of admission,the December 2013-June 2014 35 patients were treated the control group,to be discharged after routine nursing intervention;2014 July-What in October 2014 were treated 35 patients as observation group,in accordance with the principle of continuity of care for physical function assessed before hospital discharge and to develop scientific nursing program.The two groups after discharge are to be three months follow-up,and use the St.George's Respiratory Questionnaire(St George's Respiratory Questionnaire,SGRQ),12 item General Health Questionnaire(12-item General Health Questionnaire,GHQ-12),body mass index(Body Mass Index,BMI)were assessed,changes in quality of life compared two groups of patients before and after the intervention to assess the continuity of care for patients with moderate to severe COPD after discharge role and significance of the disease rehabilitation.Results: Three-month follow-up results show:1)observation group intervention effective rate 88.57%,significantly higher than 71.43%,with a statistically significant(X2=5.49,P<0.05)comparing the difference between the two groups;2)intervention groups after SGRQ scale score than before the intervention improved,SGRQ and the observation group after intervention of the sub-ratings(activity score,symptom score,impact score)were(50.71± 5.39)points,(49.74±6.55)points,(66.29±12.39)points,significantly lower than the control group,and has a significant comparative difference was statistically significant(P<0.05);after the intervention of the observation group SGRQ total score(52.32±10.47)points,and the control group intervention after the score decreased significantly compared difference between the groups was statistically significant(t = 16.76,P<0.05);3)before the intervention groups were GHQ-12 score was no significant difference in contrast,the two groups after the intervention rates were lower than the intervention before and after the intervention and observation group GHQ-12 score was(3.27 ± 2.08)points lower than the control group level,comparing scores between the two groups was statistically significant difference(t = 12.39,P<0.05);4)after the intervention control BMI index group(28.07 ± 2.18),compared with the observation group is relatively higher,compared to significant differences between the groups(t=1.84,P<0.05);and in accordance with BIM grading standards shows that after discharge patients after a series of effective interventions,observation group stage I 13 patients(37.14%),II grade 12 cases(34.28%),III grade 7 patients(20.00%),IV grade 3 patients(8.57%);BMI two groups of patients after intervention index contrast difference was statistically significant(X2 =5.49,P<0.05);FEV1 5)groups before the intervention,FVC and FEV1/FVC values contrast no significant difference(P> 0.05);after the intervention,the observation group FEV1,FVC and FEV1/FVC values were(0.51±0.09),(0.54±0.11),(0.68±0.14),relatively higher than those in the control group,compared to the differences between the two groups was statistically significant(t=5.23,7.49,8.54;P<0.05);(6)to severe quality COPD patient survival as the dependent variable,Construction of Diversified Logistics regression model to include relevant indicators for analysis,the results showed that patients in FEV1,FEV1/FVC values,BMI,are resulting in moderate to severe COPD patients survival adversely affected the quality of independent risk factors.Conclusion: By the present study shows that extended care will help to improve the quality of life in severe COPD patients after discharge,which is conducive to the rehabilitation of respiratory function and physical health of the patient also has significantly improved,the application effect is remarkable.Note that,FEV1,FEV1/FVC values,BMI,are the severe impact of the quality of life in patients with COPD risk factors in the continuation of care after discharge in need of this attention.This study will be continued as part of the extension of holistic nursing care and hospital care,so that patients can still get home sustained pulmonary rehabilitation guidance,nurse patients were repeatedly targeted health education,patients can improve their disease awareness,improve patient self-care,while nurses monitor and guide the content of discharged patients completed pulmonary rehabilitation prescription during follow-up,effectively delay the continued decline in lung function in patients,improve the quality of life of patients.
