Clinical StudyObjective: clinical research aims to use acupuncture on delivery analgesia,set up a blank control groups,electroacupuncture groups and drug analgesia groups,each groups would be conducting visual analogy scale(VAS)scores and efficacy score on every stage of before and after labor analgesia,and evaluating to maternal and child safety on electroacupuncture analgesia,so as to provide a safe and effective analgesia method for Clinical work.Methods: the purpose of this study is to compare four groups containing: it can be divided to blank control group,group of analgesic drug,dialectical acupoint group,each group has 30 cases,the group of electroacupuncture three yin and Hegu points,those would be using the random number table method.Namely: blank control,natural childbirth group;the group of electroacupuncture three yin and Hegu points;dialectical acupuncture group;drug analgesia group.Clinical observation of electro acupuncture analgesia VAS score,curative effect score,compared four groups of maternal labor process,postpartum hemorrhage volume,postpartum adverse reaction,postpartum urinary retention,neonatal asphyxia rate.Finally using SPSS19.0 statistical analysis software for statistical processing.Result:VAS score and curative effect of analgesia:Before analgesia and analgesia after 1 hour,the first labor palace mouth open seven-eight centimeter,the second stage of full dilatation,the acupuncture point group and drug analgesia group were better than the blank group,electroacupuncture group had no significant difference;Compared with the blank group,the total effective rate of electroacupuncture acupoints group and drug analgesia group was higher than in the control group,there is significant difference(P < 0.01).Birth process:Electro acupuncture group in the first stages of the active phase and the second stage of labor,the third birth process compared with the blank group can be shortening of production process,electro acupuncture group and blank group compared with statistical significance(P < 0.05);Electroacupuncture group no statistical significance(P > 0.05).Delivery outcome:Adverse reactions of four groups,postpartum urinary retention,neonatal asphyxia rate has statistical significance(P < 0.05),four groups of postpartum hemorrhage volume was not statistically significant(P > 0.05),adverse reactions in the acupuncture group was less than that of the blank group and analgesic drug group,acupuncture group no statistical significance(P > 0.05).Conclusion: The group of electroacupuncture three yin and Hegu points and dialectical acupoints group can effectively reduce labor pain,no maternal complications,can be extended to use in clinical practice.Basic ResearchObjective: To observe the effect of three yin and Hegu points groups,dialectical acupoints groups can effectively relieve labor pain,without the complications of mothers and infants,widely used in clinical.Methods: 120 successful modeling SD rats were devided into 4 groups:30 SD rats in blank group;30 SD rats in Hegu combined with Sanyinjiao point group;30 SD rats in Jiaji points group;30 parturient rats as analgesic drug group.Under the guidance of Chinese Medical theories,to locate the rat point in accordance with the practical acupuncture point map in rat,we used different electro-acupuncture points in order to ease pain in the labor beginning.With 30 SD rats as analgesic drug group,which gived remifentanil intravenous analgesia in the labor beginning.30 SD rats in blank controller group,which chosed natural delivery without take any actions.All the 4 groups of rats adopted warm water tail-flick test to determine the basic pain threshold before the labor beginning.The acupuncture groups after the labor beginning adopted warm water tail-flick test to determine the basic pain threshold at 10 min before acupuncture,and after acupuncture 10 min,20min,30 min and 60 min.Compared the threshold of pain before and after birth,and adopted the method of selecting threshold regardless of time in the maximum pain threshold in order to compare each delivery group's pain threshold variation after electric acupuncture.using real time PCR,Western blot were used to detect the brain gray matter parietal areas,spinal cord lumbar Peng Daduan and uterus muscle layer MKP-1,p38,PLA2 and PGE2 gene and protein containing amount.Finally using SPSS19.0 software for statistical analysis of the data.Results:(1)before the intervention,the pain threshold of rats in each group had no significant difference(p>0.05).(2)after the intervention: electro acupuncture group and drug analgesia group was better than control group(P < 0.05),electroacupuncture group has no significant(P > 0.05).(3)each groups of the MKP-1,p38,PLA2 gene expression in rats brain gray matter parietal areas had no significant difference(p>0.01);the level of MKP-1 gene expression was significantly higher in lumbar spinal cord expanded end through electro acupuncture group and medicine group.The expression of P38 and PLA2 in the spinal cord of the electroacupuncture group and the medicine group were significantly decreased,and the difference was statistically significant(p<0.01).The acupuncture point group and drug analgesia group were better than the blank group.Acupuncture group Hegu combine with Sanyinjiao group and Jiaji group had no significant difference(p>0.05).(4)each groups of the MKP-1,p38,PLA2 protein expression in rats brain gray matter parietal areas had no significant difference(p>0.01);the level of MKP-1 protein expression was significantly higher in lumbar spinal cord expanded end through electro acupuncture group and medicine group.The expression of P38 and PLA2 in the spinal cord of the electroacupuncture group and the medicine group were significantly decreased,and the difference was statistically significant(p<0.01).The acupuncture point group and drug analgesia group were better than the blank group.Acupuncture group Hegu combine with Sanyinjiao group and Jiaji group had no significant difference(p>0.05).Conclusions:(1)electroacupuncture at different acupoints can increase the pain threshold and has analgesic effect in rats.(2)electroacupuncture at different acupoints can increase the expression of MKP-1 and protein in the spinal cord,and no significant change in the gray matter of the brain.Electroacupuncture at different acupoints can decrease the expression of P38,PLA2 and PGE2 in the spinal cord and the uterine muscular layer,and there is no significant change in the gray matter of the parietal lobe of the brain.
