Part I: Clinical researchObjective : To observe the different electrical points for delivery of maternal and neonatal impact,in order to reveal the molecular mechanisms of their labor analgesia from the MAPK/ERK signaling pathway to explore a non-drug,simple,effective and safe for mother and child,no labor analgesia injury,for the application of electric needle to provide a scientific basis for labor analgesia.Methods:120 patients met the inclusion criteria of maternal childbirth,randomly divided into four groups: blank control group,dialectical acupoints EA group,San yinjiao combined with Hegu EA group,drug analgesia group,n=30,make the appropriate treatment and control was observed for the electrical impact of maternal and newborn.Record labor observe the progress of the first stage of labor,understand the analgesic effect of acupuncture treatment,the application volume assay 2h postpartum vaginal bleeding reference level IV pain judging method and the VAS score and apply the law of the Apgar score after birth 1 minute,5 minutes,10 minutes regular rates.All data were SPSS20.0 statistical analysis software.Results:1.The first stage of progress:(1)Incubation period: the incubation period of two EA groups and drug analgesia group was significantly shortened(P<0.01),in descending order:blank control group>dialectical acupoints EA group/San yinjiao combinedwith Hegu EA group>drug analgesia group.(2)Active period: the active period of two EA groups and drug analgesia group was significantly reduced(P<0.01),in descending order: blank control group>dialectical acupoints EA group/San yinjiao combined with Hegu EA group>drug analgesia group.2.Analgesic effect:(1)The total effective rate of acupuncture group and two groups of maternal labor analgesia analgesia was significantly higher(P<0.01),in descending order: drug analgesia group>dialectical acupoints EA group/San yinjiao combined with Hegu EA group>blank control group.(2)Two electric acupuncture groups and drug analgesia group maternal VAS pain score after 1h,cervix 7-8cm,when the whole cervix were significantly lower(P<0.01),in descending order: blank control group>dialectical acupoints EA group/San yinjiao combined with Hegu EA group>drug analgesia group.3.The amount of postpartum hemorrhage: two electric acupuncture groups decreased postpartum 2h vaginal bleeding volume(P<0.01),drug analgesia group postpartum 2h vaginal bleeding was not statistically significant(P>0.05),four groups of postpartum 2h vaginal bleeding in descending order:blank control group/drug analgesia group>dialectical acupoints EA group/San yinjiao combined with Hegu EA group.4.Apgar score: EA group after two birth mothers produce 1 minute,5minutes Apgar scores were significantly increased(P<0.01),analgesia groups were produced fetus born after 1 minute,5 minutes Apgar score was not statistically significant(P>0.05),after the baby was born 1 minute,5 minutes Apgar scores in descending order: dialectical acupoints EA group/San yinjiao combined with Hegu EA group>blank control group/drug analgesia group.After each group of maternal birth produced a 10-minute Apgar scores were not statistically different(P>0.05).Conclusion:1.Effect of maternal: electro-acupuncture can relieve labor pain,shortening the first stage of labor and reduce postpartum 2h vaginal bleeding.2.Impact on the newborn: normal childbirth application that acupuncture can reduce the incidence of neonatal asphyxia.Part II: Basic researchObjective:To investigate the MAPK/ERK signaling pathway different electrical acupoint analgesia for labor in rats,and to observe the gray matter parietal area of the brain after birth,enlarged lumbar spinal cord segment signal molecule Raf,ERK,CREB's gene and protein expression and the protein expression of myometrium of effector molecules DYN reveal underlying mechanisms of acupuncture analgesia from cellular and molecular level,to provide a safe,effective and simple treatment for clinical analgesia,but also for electricity for the town of childbirth pain further research to provide scientific and theoretical basis.Methods : 120 cases of successful modeling pregnant SD rats were randomly divided into four groups: blank control group,acupuncture Jiaji group,acupuncture San yinjiao combined with Hegu group,drug analgesia group,n=30.Rats were measured with hot flick delivery method when pain threshold,and sensing applications Western blot,Real-time PCR and other modern molecular biology techniques four groups of rats parietal gray matterarea,enlarged lumbar spinal cord segments MAPK/ERK pathway signaling molecules Raf,ERK,CREB's protein and gene expression and the protein expression of myometrium of effector molecules DYN.All data SPSS20.0statistical software for statistical analysis.Results:1.The pain threshold aspects: Before treatment,the pain threshold of the rats during childbirth was no significant difference(P>0.05);After treatment,the two groups and EA analgesia group childbirth pain threshold of rats were significantly increased(P<0.01),in descending order: drug analgesia group>Jiaji group/San yinjiao combined with Hegu group>blank control group.2.The terms of gene expression: the m RNA expression content of Raf,ERK,CREB is no significant difference(P>0.05)in the brain of rats;two EA groups and drug analgesia group can significantly reduce the m RNA expression content of Raf,ERK,CREB in the rat spinal cord(P<0.01),the expression of content in descending order: blank control group>Jiaji group/San yinjiao combined with Hegu group>drug analgesia group.3.The terms of expression: the protein expression content of Raf,ERK,CREB is no significant difference(P>0.05)in the brain of rats;two EA groups and drug analgesia group can significantly reduce the protein expression content of Raf,ERK,CREB in the rat spinal cord(P<0.01),in descending order: blank control group>Jiaji group/San yinjiao combined with Hegu group>drug analgesia group;two EA groups and drug analgesia group could significantly increase the protein expression content of DYN in the myometrium tissue of rats(P<0.01),in descending order: drug analgesia group>Jiaji group/San yinjiao combined with Hegu group>blank control group.Conclusion:1.EA extended childbirth pain threshold of rats for analgesic effect.2.The electrical stimulation can reduce m RNA and protein expression content of signaling molecule Raf,ERK,CREB in the rat spinal cord,increase protein expression of effector molecule DYN in the myometrial tissue.3.EA mainly through the MAPK/ERK signaling pathway regulate and control the expression of signaling molecule Raf,ERK,CREB in the rat spinal cord,thereby regulating the effector molecules DYN to produce analgesic effect.
