Objective: to study the effects of controlled epidural analgesia on labor and delivery outcome and newborns.Methods: retrospectively study the 520 term primiparaes who had a natural delivery in the First People’s Hospital of Suzhou between January and September in 2015. 170 of them had been performed analgesia delivery, inlcluding 18 who suffered from pregnancy-induced hypertension and mild preeclampsia. According to the inclusion criteria and exclusion criteria, they can be divided into two groups: the observation group and the control group.On two groups of the labor time, oxytocin utilization rate, rate of postpartum hemorrhage, fetal distress, VAS score, delivery mode comparison.Result:1. The observation group of the first "active" labor significantly shorter than the control group, the difference was statistically significant(P < 0.05). Observation group’s second labor time was significantly longer than the control group, the difference was statistically significant(P < 0.05). Oxytocin usage observation group was obviously higher than that of control group, the difference was statistically significant(P < 0.05). Postpartum hemorrhage, neonatal asphyxia, forceps midwifery has no statistical significance(P >0.05).2.The control group had a case of failure, who had been terminated pregnancy by operating because of the strong contractions.3. The difference between the first and second labor time of group at different ages was statically significant.( P<0.05) But there is no statically difference between the observation group and the control group whose age was between 25 and 28.(P>0.05)4. There were less epidural anesthesia in the hypertension group, with a number of 18.This group had no control group. The blood pressure of pregnancy was stable and delivered smoothly. Only 1 case had a dramatic blood fluctuation during the second period.And we deduced that the epidural anesthesia has favorable influence on pregnancy with high blood pressure.Conclusion:1. Analgesia labour can obviously relieve labor pain and significantly shortened the first "active" labor, but significantly increased the second labor time, does not increase the rate of cesarean section, forceps midwifery rate, rate of postpartum hemorrhage and neonatal asphyxia rate, and does not affect the lower limb motor nerve, but oxytocin utilization rate significantly increased. This method is good effect, less side effects, the method is simple, safe and reliable.2. There was a failure case in this group. And the failure may had something to do with the pain sensitivity or the anesthetic tolerance of the pregnancy. Even it may had something to do with the skills of the anesthetist. So the PCEA also has its limitation.3. During the analgesia delivery, the second labor time was not extended and the first labor time was shortened in the group whose age was between 25 and 28.4. The analgesia delivery may have an effect of stabling the blood pressure during the delivering. It helps to reduce the fluctuation of blood pressure.
