Part?.Study on the measurement of labor pain and its related factors.[Abstract]Objective To determine the pain score during childbirth and to explore the factors associated with childbirth pain.Methods Parturients who selected viginal delivery from March to November,2017,were recruited.Continuous measurements of labor pain were conducted in time.The labor pain related comprehension questionnaire and General Self-Efficacy Scale(GSES)and Hospital Anxiety Scale(HAS)were used.The descriptive statistical analysis,single factor analysis,Pearson correlation analysis and multiple logistic regression analysis were conducted using Statistical Product and Service Solution(version 20.0).Results The labor pain score of every cervix showed a trend of rising.The labor pain was severe when the cervix were at 6?7 cm.The mean labor pain score was(9.09±0.68).The multiple logistic regression analysis revealed that the parity,self-efficacy and the initiation time of labor pain were the influencing factors of labor pain.Conclusion Labor pain was a severe pain.The medical personnel should take appropriate intervention measures according to the factors associated with the labor pain to reduce the pain score,raise the pain threshold,improve the quality of delivery and culture and to optimize the outcome of childbirth.Part?.Comparative study on different birth analgesic measures.? The comparative analysis of analgesic effect of epidural analgesia at different time of first stage of labor.Objective To explore the effect of epidural analgesia at different time of first stage of labor.Methods 133 parturients were selected for the study,62 were selected for epidural analgesia at latent period(the latent group),and 71 were selected for epidural analgesia at active period(the active group).The author observed and compared the scores of labor pain in different labor stages,the degree of motor block in the second stage of labor,and the delivery satisfaction and outcome of childbirth.Results For the latent group,the pain score at latent period,at the 3-5 cm cervical opening and during the whole stage of labor were 3.01?6.83?3.61,respectively that were lower than those in active group(7.11?6.83?5.23).The first stage of labor time(402min)?the rate of instrumental delivery(14.5%)and the score of delivery satisfaction(83.71)were superior than those of the active group(513min?23.9%?61.22 respectively),of which the differences were statistically significant(P<0.05).Conclusion Adopting the epidural analgesia at latent period has obvious analgesic effect,and it could improve the parturients' labor satisfaction,shorten the first labor stage time,reduce instrumental delivery rate and improve the delivery quality.? Comparative study on the effects of Transcutaneous electrical nerve stimulation and the epidural analgesia.Objective To compare and analyze the effects of transcutaneous electrical nerve stimulation(TENS)and epidural analgesia on labor pain,and to provide a theoretical basis for the choice of childbirth analgesia.Methods Primiparas hospitalized in a Grade ? Class A hospital for delivery in Shandong Province were selected and divided into three groups according to the analgesia methods:the group of transcutaneous electrical nerve stimulation analgesia(TENS)?the group of epidural analgesia and control group.The author observed and compared the scores of labor pain in different labor stages,the degree of motor block in the second stage of labor,the delivery satisfaction of the women and the outcomes of childbirth for three groups.Results The pain scores at 5-6 cm?6-8 cm?8-10 cm of cervical opening in TENS group and epidural group were all lower than those of control group(P<0.01).The pain score at 5-6 cm of cervical opening in TENS group was similar to that of the epidural group,at which the difference was not statistically significant.However,the pain scores at 6-8 cm?8-10 cm of cervical opening in two group are significantly different(P<0.01).The delivery satisfactions of the two groups(which were 65.23,66.57 respectively)were higher than that of the control group(49.36).The time of the first stage of labor in TENS group and epidural group(which were 236 min and 228 min respectively)were shorter than that of the control group(280 min);the time of the second stage of labor in epidural analgesia group(which was 69 minutes),the score of motor block(which was 0.11)and the rate of instrumental delivery(which was 33.3%)were all superior than those of TENS group(which were 38min,0.11,18%respectively)and those of the control group(which were 45min,0.09,18.4%respectively.The time of the third stage,the rate of cesarean,the scores of 1 min Apgar and the postpartum bleeding were all not significant different between 3 groups(P>0.05).Conclusion TENS analgesia and epidural analgesia all had obvious analgesic effect.Transcutaneous nerve stimulation analgesia has the advantages of mild motor block,shortened first stage of labor,no extension of second stage of labor,and no increase in rate of instrumental delivery,and it is a non-drug,non-invasive,simple and easy to use analgesic method,which was economically safe,thus worth popularizing in clinical practice.
