Objective: Through the comparison of the effect of labor analgesia instrument and labor in water on labor quality of normal primipara,this paper aims to guide primipara and obstetricians better to choose and recognize the non-drug analgesia,so as to improve the rate of vaginal delivery.Methods: A total of 645 women who is healthy,monocyesis,primary parturient and underwent vaginal trial labor in Gansu provincial people's hospital from December 2018 to December 2019 were selected as the study objects.According to the choice of different analgesia methods of pregnant women and their families,they were divided into three groups:the group of who use labor analgesia instrument included 93 cases,the group of labor in water included 284 cases,and the control group of who never used labor analgesia included 268 cases.Collect the time of every stage of labor,the rate of episiotomy,the amount of postpartum bleeding,the rate of postpartum bleeding,the neonatal Apgar score,the rate of neonatal asphyxia,the cesarean section rate and the puerpera's pelvic floor functional at 42 days after delivery.All data were analyzed by SPSS 25.0 software.Results: Compared with the group who used labor analgesia instrument and the control group,the group who labor in water reduced the time of total stage of labor and the time of first stage of labor,the difference was statistically significant(P<0.05);Compared the time of total stage of labor and the time of first stage of labor between the group who used labor analgesia instrument and the control group,the difference was not statistically significant(P>0.05);Compared the time of second stage of labor,the time of third stage of labor,the rate of episiotomy,the amount of postpartum bleeding,the rate of postpartum bleeding,the neonatal Apgar score and the rate of neonatal asphyxia among the three groups,there was no statistically significant difference(P>0.05).The cesarean section rate of the group who labor in water was lower than the group who used labor analgesia instrument and the control group,and the difference was statistically significant(P<0.01);Compared the cesarean section rate between the group of who used labor analgesia instrument and the control group,the difference was not statistically significant(P>0.01);Compared with the incidence of pelvic organ prolapse at 42 days after delivery among the three groups,the rate of the group who labor in water was lower than that the group of who used labor analgesia instrument and the control group,and the rate of the group of who used labor analgesia instrument was lower than that the control group,the difference was statistically significant(P<0.01).Compared the muscle strength of type I and type II muscle fibers in the pelvic floor tissues among the three groups,the group who labor in water was superior to the group of who used labor analgesia instrument and the control group,and the group of who used labor analgesia instrument was superior to the control group,the difference was statistically significant(P<0.01).Conclusion: Labor in water can shorten the time of first stage of labor and reduce the rate of cesarean section;Using labor analgesia instrument and labor in water don't prolong the time of second and the time of third stages of labor;Using labor analgesia instrument and labor in water have no obvious adverse effect on the rate of episiotomy and the amount of postpartum bleeding;Using labor analgesia instrument and labor in water have no obvious adverse effect on the neonatal Apgar score and neonatal asphyxia;Using labor analgesia instrument and labor in water can reduce the damage of labor to pelvic floor tissue,and labor in water is better than Using labor analgesia instrument.
