1 ObjectiveBy reaserching changes of the immune system, expecially the expression of TNF-?and IL-1? in serum, of the adjuvant arthritis(AA) rats, to evaluate that how they were affected by the modified Chengshijuanbi decoction. It was designed to investigate the mechanism to the modified Chengshijuanbi Decoction for how it improved the immune system in Rheumatoid arthritis(RA).2 Methods60 male SD rats were randomly divided into the four following groups: normal control group(NC), model control group(MC), tripterygium glycosides tablet treatment group(TGT) and modified Chengshijuanbi Decoction treatment group(JBT), with 15 rats in each group.Except for the normal control group, each rat of this study was given Freund's complete adjuvant via intracutaneous injection in right rear vola pedis to copy AA rat model; On the first day after model replication, except the normal group, the rest rats were placed in cold water(depth of 2 cm) for 20 min. And at the same time, wind of level 3 was added once daily for consecutive 14 days to copy the arthralgia disease of wind-clod-dampness type.On the 15 day after modeling, namely the end of the model replication, the rats were given medication. JBT group was given gavage of 9ml/kg of Chengshijuanbi Decoction(8 times clinical medication of adults of 60 kg) once daily, equal to 137 g crude drugs(crude drug content of 2 g/ml); TGT group was given gavage of 10ml/kg of Tripterygium Glycoside suspension(Tripterygium Glycoside Tablet was grinded into powder and added with saline to prepare suspension, and per ml of suspension had 0.57 mg of Tripterygium Glycoside Tablet) once daily; the model group and the normal group were given gavage of 10 ml/kg of saline once daily. All rats were continouously dosed oner per day for four weeks. Observe the activity of the AA rats, paw swellingand arthritic index(AI), and observe the expression of TNF-? and IL-1? in serum by using ELISA; meanwhile, synovium tissue pathological changes of the the rats' joint were obsevered under the microscope.3 Results3.1 Effect of JBT for AA rats' general pathological changesModel rats appeared swelling and redness in the left rear paw on the next day of inflammation-induced treatment, and had different degrees of swelling and redness in the right rear paw about 5 days later. Then the rats appeared slight redness and swelling in the front paw, also ulceration and crusts in the tail roots, canthus and the like. Rats with serious restricted movement were unable to bear load. Compared with the normal control group, rats of model group had obviously lower activity, depressed spirit, poorer appetite, loose stool slow growth of body weight and even reduced body weight. After modeling, rats of the treatment group had obviously improved redness and swelling in the front paw 15 days after medication. The rear paw was improved later, and the activity was raised compared with the model group. There was no abnormal change of the rat joint compared with the normal control group.3.2 Effect of JBT for paw edema of the AA ratsRats of MC group showed no significant differences in paw before sampling and before giving saline(P>0.05); rats of TGT group had obviously improved swelling in paw before sampling, which was significantly different from that before medication(P<0.01). After treatment, rats of JBT group had obviously improved swelling in paw before sampling, which was very significantly different from that before medication(P<0.01). Compared with MC group, rats of TGT group showed significant differences in paw swelling before sampling(P<0.05); Compared with MC group, rats of TGT group showed very significant differences in paw swelling before sampling(P<0.01).3.3 Effect of JBT for AI of the AA ratsRats of MC group showed no significant differences in AI before sampling and before giving saline(P>0.05); rats of TGT group had obviously improved in AI dgreebefore sampling, which was very significantly different from that before medication(P<0.01). After treatment, rats of JBT group had obviously improved in AI dgree before sampling, which was very significantly different from that before medication(P<0.01).Compared with MC group, rats of TGT group showed differences significant in AI dgree before sampling(P<0.05); Compared with MC group, rats of TGT group showed very significant differences in AI dgree before sampling(P<0.01).3.4 Effect of JBT for articular pathological in AA ratsThe synovium of the rats in NC group cannot be found any imflammatory cells infiltration, while compared with NC group, it can be found a large number of imflammatory cells and multiple vasculars hyperplasis in the synovial layered in the synovium tissue of the rats in MC group; on the contrary, after treatment, compared with MC group, the number of the inflammatory cells and multiole casculars hyoerplasis in synovium of the rats in JBT goup were decreased obviously.3.5 Effect of serum cytokines TNF-? in AA rats after being treated by JBTCompared to NC group, serum TNF-? level in rats of MC group obviously increased, which was very significantly different(P<0.01). Rats in TGT group showed very significantly differences from that in rats of NC group(P<0.01), but no differences from that in rats of MC group(P>0.05). Serum TNF-? level in rats of JBT group obviously decreased, which showed no significantly difference compared to rats in NC group(P>0.05), while there was very significantly difference from that in rats of MC group(P<0.01).3.6 Effect of serum cytokines IL-1? in AA rats after being treated by JBTCompared to NC group, serum IL-1? level in rats of MC group obviously increased, which was very significantly different(P<0.01). Rats in TGT group showed no differences from that in rats of NC group(P<0.01), but very significantly differences from that in rats of MC group(P>0.05). Serum IL-1? level in rats of JBT group obviously decreased, which showed no significantly difference compared to rats in NC group(P>0.05), while there was significantly difference from that in rats of MC group(P<0.05) and no significantly difference compared to rats in TGT group(P>0.05).4 Conclusion4.1 JBT can improve the general pathological change on AA rats, and has no adverse effect on the body weight growth and the survival rat of the AA rats;4.2 JBT can improve the paw swelling and reduce arthritis index on AA rats;4.3 JBT can reduce the level of the serum TNF-? and IL-1? on AA rats;4.4 JBT can improve the articular pathological, which contains the number of the inflammatory cells and multiole casculars hyoerplasis in synovium, on AA rats of the rats.
