The Postoperative Trauma And Pain In Patients: Da Vinci S Surgical System Vs VAMT

ObjectiveTo compare and analysis the effect of Da Vinci S Surgical System and video-assisted mini-thoractomy(VAMT)on postoperative trauma and painMethodsTo analyze the 88 cases with lungs tumor who underwent surgical treatment from Jan.2015 to Apr.2015,39 of which with VAMT and 49 with da Vinci S Surgical System.We collected the datas of operating time,time of drainage,the scores of pain at postoperative 24 hours,the scores of pain at postoperative 48 hours,WBC count and the contents of IL-6 and TNF-?.the serum Concentration of IL-6 and TNF-? is indicator as a surgical trauma to the body.To compare serum interleukin 6(IL-6)and tumor necrosis factor-?(TNF-?)differences before and after operation.SPSS19.0 statistical software for statistical analysis.Measurement data conforms to normal distribution statistical description in mean + /-standard deviation by independent sample T-test.Count data by analysis of variance with p<0.05 designated as significance.ResultsSurgical time: robot group(131.41±56.38)mins,VATS group(158.77±54.41)mins(t=-2.298,P=0.024);time of drainage: robot group(6.41±5.77)d,VATS group(8.85±3.61)d(t=-2.421,P=0.018);the VAS scores of postoperative 24 hours:robot group(2.00±0.74)scores,VATS group(3.33±1.64)scores,t=-4.704,P=0.000;the VAS scores of postoperative 48 hours:robot group(1.71±1.00)scores,VATS group(1.77±1.11)scores,(t=-0.244,P=0.808);WBC:preoperative,robot group(7.03±1.15)×109/L,VATS group(7.38±1.46)×109/L,(t=-1.085,P=0.281);postoperative,robot group(8.16±2.09)×109/L,VATS group(11.74±3.48)×109/L,(t=-5.658,P=0.000);comparison of preoperative and postoperative in robot group,t=-4.292,P=0.000;comparison of preoperative and postoperative in VATS group,(t=-7.343,P=0.000);IL-6:preoperative,robot group(2.28±1.30)pg/ml,VATS group(2.83±1.42)pg/ml,(t=-1.901,P=0.061);postoperative,robot group(6.75±4.10)pg/ml,VATS group(21.86±18.00)pg/ml,(t=-5.138,P=0.000);comparison of preoperative and postoperative in robot group,(t=-7.775,P=0.000);comparison of preoperative and postoperative in VATS group,(t=-6.790,P=0.000).TNF-?:preoperative,robot group(0.75±0.68)pg/ml,VATS group((0.77±0.62))pg/ml,(t=-2.090,P=0.048);postoperative,robot group(1.90±1.10)pg/ml,VATS group(2.86±1.20)pg/ml,(t=-6.246,P=0.000);comparison of preoperative and postoperative in robot group,(t=-6.586,P=0.000);comparison of preoperative and postoperative in VATS group,(t=-7.680,P=0.000)Conclusion Compared with VATS,The da Vinci S Surgical System can be safer used for the treatment of lungs tumor with shorter operating time,shorter time of drainage,less pain,less trauma.
