As the health-care industry is of high-risk, there is potential risk throughout each link of the operation of it. As we all know, human is the most important part of medical system which is of high requirements of reliability, safety and precision. Saying "to err is human", making mistakes is unavoidable, and we found that almost every question will be out in the human body itself in the end. So human error is the biggest hidden danger which can cause the medical risk and threat to patient safety. So we urgently need to introduce HRA and Human Error theory into the medical service field to reduce avoided man-made risk and to improve patient safety.Human error is not only the cause of medical adverse, which may also increases the severity of the consequence to the adverse. From two dimensions of “cause-effect” the paper identifies how human errors cause the event, and explores how human errors of medical staff in dealing with the every stage of the accident affect the severity of the consequence to the adverse and reliability of the system at the same time in perspective of medical care process.Based on the achievements of related research and theory at home and abroad, we put forward a set of general human risk analysis method for emergency medical adverse combining with the characteristics of the medical services industry in the paper. What`s more, it takes hemodialysis accident-air embolism as an example, combing FTA with ETA, the paper divides the thread of what medical staff do after the adverse happened into several nodes with “big event tree”, and with these nodes as the top event,set up “small fault tree” by taking human factor faults as the core. Finally the paper identifies human factor fault of the key risk nodes, and work out quantitative evaluation indexes like failure rate of top event in the failure tree ?the reliability of the different accident sequence in the “big event tree”.
