ObjectiveThe study was to understand the self-perceived burden of patients with maintenance hemodialysis.Analysis of general demographic data and disease data influence on maintenance hemodialysis patients' self-perceived burden.Analysis of maintenance hemodialysis patients' self-perceived burden and self-management behavior,self-esteem,family concern degree of correlation.Analyze the main factors related to the self-perceived burden of patients of maintenance hemodialysis.MethodsThe study used convenience sampling questionnaire method,386 patients with maintenance hemodialysis,who were from hemodialysis rooms of the two hospitals in Dalian city and two hospitals in Jinzhou city from October 2014 to May 2015,investigated by general demographic questionnaire,self-perceived burden scale,self-management behavior scale,self-esteem scale,family APGAR index scale,Finally,369 valid questionnaires were obtained.The data had received were analyzed using SPSS19.0 statistical software,the following analysis methods included descriptive statistical analysis,t test,one-way variance analysis,Kruskal-Wallis H test,correlation analysis and regression analysis.ResultsThe scores of self-perceived burden scale was(35.26±7.43)scores,and 91.60% patients had existed self-perceived burden in different degree,Among them,no apparent self-perceived burden accounted for 8.4%(31 cases),mild self-perceived burden attributable to 15.99%(59 cases),moderate self-perceived burden attributable to 43.36%(160 cases),severe self-perceived burden accounted for 32.25%(119 cases).In terms of the average scores of dimension,economic burden was the highest(3.85 scores),followed by the emotional burden(3.70 scores),body burden was the lowest(3.06 scores).The result of statistical analysis showed,marriage(F=26.405,P=0.000),family monthly income(X2 =177.54,P=0.000),medical payment(F =4.242,P=0.015),health condition of the caregivers(X2 =140.494,P=0.010),self-care ability of parents(X2 =152.082,P=0.000),the age of dialysis(X2 =41.033,P =0.000)of self-perceived burden of patients' general demographic characteristics was statistically significant(P<0.05).The correlation analysis showed that the patients' self-perceived burden scale scores and self-management behavior scale scores,which had a negative correlation(r=-0.535~-0.175,P<0.05),which and self-esteem scale scores showed a negative correlation(r=-0.320,P<0.05),which and family APGAR index scale showed a negative correlation(r=-0.292,P<0.05).The scores of self-perceived burden scale as thrdependent variable,age,marriage,family income,medical payment,self-care ability,physical condition of caregivers and the self-esteem scores,age of dialysis,self-management behavior scale scores,family care index subscale and total scores entered the regression equation(F=88.661,P<0.05.standardized partial regression coefficient b =-0.179,-0.097,-0.238,0.849,-0.311,-0.135,-0.141,0.140,-0.238,-0.099),and R2=0.704.ConclusionsMaintenance hemodialysis patients had self-perceived burden obviously and high rate,which was affected by a variety of factors,including self-management behavior,family monthly income,self-care ability,age,self-esteem,the age of dialysis,the health of caregivers,family APGAR index,marriage,medical payment.By the standard regression coefficient can be seen,the family monthly income,self-management behavior and self-care ability had a more obviously effect on the self-perceived burden of partents of large,suggesting that the nurses can proceed from these aspects,it can provide reference basis for take the measures to reduce maintenance hemodialysis patients' self-perceived burden in the future.
