ObjectiveTo investigate the current burden and quality of life of family member who undertake the main nursing for patients with maintenance hemodialysis.Analyze current burden correlation with their quality of life.Explore the burden influencing factors of caregiver.It will provide reference for intervention strategies to improve the quality of life of the patients and their main caregivers.MethodsObservation of convenient samples were performed in this study.Maintenance hemodialysis diabetes and their primary caregivers from endocrine department of A 3 grade hospital in Kaifeng city were investigated.The investigation lasted for 9 months from September 2016 to June 2017.This study adopted the method of questionnaire survey,to understand the basic condition of the hemodialysis patients and their family caregivers through general information;to assess the Burden of family Caregivers of patients with hemodialysis through the caregiver Burden questionnaire(The family burden scale of disease,FBS);to investigate quality of life in hemodialysis patients through the Mos 36-item Short Form Health Survey.The data were established by Epidata3.0 software.Statistic methods included descriptive analysis,variance analysis,multiple linear step wise regression analysis and Spearman correlation analysis,Pearson correlation analysis were performed by the IBM SPSS Statistics 22.0.Results1.210 questionnaires were distributed,among which 204 were retrieved and valid,204 finished by patients and 204 finished by their caregivers.The recovery rate was 97.1%.The total family burden of the caregivers of hemodialysis patients was(15.01±7.47),with 16 cases of mild cases,7.8 percent of all patients and 93 cases of moderate level,accounting for 45.6 percent.The severe level was 95,accounting for 46.6%.The results showed that the score with the highest score was the economic burden,with the score(5.48±3.56).Secondly,the daily life burden,score(4.05±3.37),This was followed by the burden of recreational activities(2.48 ± 1.95),mental health(0.89 ± 1.04),family relations(1.57 ± 2.22)and physical health(0.54 ± 1.02).The patient's own characteristics affect caregiver burden include: maintenance 45.6 percent.The severe level was 95,hemodialysis patients with time,self care,medical treatment charge way and presence of complications were statistically significant(P<0.01)or less.The factors that influence the care burden include the number of years of care,the time of day care,the age of the caregiver,and whether there is a nanny(P <0.01).2.It was showed by multiple linear stepwise regression analysis that the duration on hd,patients' selfcare ability,the primary caregivers' age,attending time limit,take care of time every day,family in come whether suffered chronic diseases and subjective support were included in the regression equation(F=12.137,P=0.000)in which the dependent variable was caregivers' burden.The 6 factors could explain 53.9% of the variation for the caregivers' burden.3.With SF-36 for MHD patients to measure the quality of life of caregivers,and the results show that compared with the general population,the quality of life of caregivers generally low,in addition to mental health dimension has no difference with the general population(P=0.057),the rest of the dimensions have significant difference(P <0.01)or less.The scores of each dimension were as follows: physiological function(59.22 ± 21.62),physiological function(50.85 ± 31.81),physical pain(62.32 ± 24.86),energy(58.18 ± 16.69),social function(58.08 ± 25.94),mental health(63.51 ± 15.63).The two factors with the lowest score were health status(47.13 ± 25.62)and emotional function(41.34 ± 28.38).4.Through Spearman correlation analysis,the conclusion showed that the burden and quality of life of main caregivers of patients with maintenance hemodialysis was negatively related(P<0.05,P<0.01).Conclusion1.The burden of primary caregivers of patients with maintenance hemodialysis was influenced by the duration on hd,the patients' self-care ability,the primary caregivers' age,attending time limit,take care of time every day,family in come were the main influencing factors.2.The primary caregivers' caregiver burden and QL were negatively correlated.The lower caregiver burden the higher quality of life was.To sum up,the key to improve the quality of life was reduced the burden of caregiver.
