Objective1. To explore the self-perceived burden status of the patientswith advanced cancer.2.To identify the impact factors on the self-perceived burden andanalyze the interac tions between various factors, to provide a theoreticalbasis for the clinical implementation of specific nursing interventions.MethodThe self-designed basic information questionnaire, Self-PerceivedBurden Scale, the Patient Dignity Inventory, Distress Thermometer andEdmonton Symptom Assessment Scale were used to investigate299patients with advanced cancer in Shandong province. Firstly, Generaldescriptive statistics, single factor analysis, multiple regression analysis wereused to analyze the self-perceived burden and its influencing factors, then themajor ones were identifyied. Secondly, path analysis was used to analyzethe interactions among influencing factors of the self-perceived burden.Results1. The average score of the self-perceived burden patients with advancedcancer was25.63±8.708, which arrived at the moderate level.2. The self-perceived burden was influenced by gender, residence,occupations, empl-oyment status, education level, caregiver, social support,family harmonious, insurance,self-care and metastasis.3. The self-perceived burden was influenced by psychological distress,practical conc-erns including child care, hoursing,work or study, andfinancial distress, emotional concerns including depression, anxiety, worry,fear, nervousness, sadness, loss of interest in things, family concernsincluding dealing with children and partner.4. The self-perceived burden was influenced by loss of dignity. 5. The self-perceived burden was influenced by physical concernsincluding pain, fati-gue, nausea, sleep, appetite, shortness of breath,appearance, constipation, indigestion, tingling in hands/feet.6. The main factors affecting the self-perceived burden of the patientswith advanced cancer were loss of dignity, financial distress, occupations,psychological distress, child care, the quality of life, fatigue.7. Path analysis showed that psychological distress and the quality of lifecould affect the self-perceived burden of the patients with advanced cancer,the quality of life directly,occupations could affect it indirectly through thequality of life and loss of dignity, fatigue could affect it indirectly through thequality of life, child care and loss of dignity could affect it indirectly throughthe psychological distress, financial distress could affect it directly andindirectly.Conclusion1. This study primarily verified that the the SPBS, PDI,ESAS had good reliability an-d validity, and it could be futher validated andapplied widely. 2. The self-perceived burden among patients with advanced cancerarrived a mod-erate level, its incidence was81.3%(the division of SPB totalscore≥18). The incidence of psychological distress was64.21%, and theincidence of loss of dignity was30.10%.3. Factors affected the self-perceived burden among patients withadvanced cancer were gender, residence, occupations, employment status,education level, caregiver, social support, family harmonious, insurance,self-care and metastasis, psychological distress, practical concerns, emotionalconcerns, physical concerns, the quality of life and loss of dignity.4. Path analysis showed that financial distress was the most importantinfluencing factors, followed by the quality of life, psychological distress, lossof dignity, fatigue,child care, occupations. Financial distress could affect theself-perceived burden of the patients with advanced cancer directly andindirectly, the quality of life and psychological distress could affect itdirectly,occupations,loss of dignity, fatigue, child care could affect itindirectly.
