Analysis On Independent Relative Factors Of Chronic Subjective Dizziness

Background/Objectives:Reviewing studies about chronic subjective dizziness (CSD) found a lot of researches concerned about the primary symptoms, provoking or aggravating factors, identifying and diagnosis, common complications and probable treatment of CSD. A few hypotheses have been proposed concerning its maintenance and trigger mechanism. However, few involved in the independent relative risk factors of CSD. Therefore, it is necessary to study the basic clinical documents to compare CSD patients and normal controls in gender and age so that to find out the possible relative factors.Methods:107 diagnosed CSD patients were screened out from the continuous outpatient registration and have completed the test scales in the Department of Neurology, Yangzhou No.1 People's Hospital. Control group is made up of 157 participants inspected as health by the hospital and have completed the test scales in the corresponding period. All participating individuals signed the Information Consent approved by the Medical Ethics Committee of Yangzhou No.1 People's Hospital. Beside their gender and age, the following history were registered, namely, hypertension, diabetes, cervical spondylosis, hyperlipemia, insomnia, regular interval venous infusion, migraine associated vertigo, vertebrobasilar artery insufficiency, history of stroke, previous acute vestibular disorder, mild traumatic brain injury, vestibular neuronitis, panic attacks or generalized anxiety, depression. Some blood values,such as white blood cell count, hematocrit and fibrinogen, were included in the registration. Further information collected included the Hamilton Depression Scale-14 (HAMD,14 item) and Hamilton Anxiety Scale-24(HAMA,24 item) and Pittsburgh Sleep Quality Index Scale(PSQI). The t test and ?2 test were carried out on the two groups for statistical analysis and multiple regression equation was applied to predict the CSD independent relative factors.Result:No significant difference (p>0.05) was found between CSD disease group (n=107) and the control group (n=157) regarding 17 basic aspects below:gender, age, history of hypertension, diabetes, cervical spondylosis, hyperlipemia, migraine associated vertigo, vertebrobasilar artery insufficiency, history of stroke, mild traumatic brain injury, vestibular neuronitis, panic attacks or generalized anxiety, depression, white blood cell count,hematocrit value and fibrinogen. Compared with the control group, CSD group had higher rate of previous acute vestibular disorders (n=40,37.4% vs 9.6%, P=0.000) and regular interval venous infusion (n=47,43.9% vs 14.0%, P=0.000) and insomnia (n=85,79.4% vs 25.5%, P=0.000).Logistic regression analysis showed that previous insomnia (OR 10.020,95% CI 5.202-19.302; P=0.000), regular interval venous transfusion (OR4.181,95 % CI2.014-8.680; P=0.000) and previous acute vestibular disorders (OR4.68 9,95% CI 2.096-10.491; P=0.000) were independent relative factors of CS D.Conclusion:Previous acute vestibular disorders, regular interval venous infusion, insomnia are the independent relative factors of CSD.
