The Development Of A Medical Message Pushing System Based On Pub/Sub Mechanism

The medical message pushing systems are widely used in clinical settings. They push messages to health workers and remind them to execute medical actions in time, which is an important way to ensure medical safety and improve quality of care. However, those systems did not consider the personalized demand for messages in actual clinical setting, they push the same messages to health workers in the same area, which lead to serious remind fatigue problem.To solve the problem, this thesis proposes a medical message pushing system based on pub/sub mechanism. The system pushes messages according to the subscription of health workers, which not only meets the personalized message needs but also decreases the interruption of irrelevant messages. There are mainly three tasks:1) We designed a topic-content based message model by analyzing the sources of medical messages. The message model provides a general semantic structure for messages from heterogeneous publishing systems. Furthermore, we also designed a topic-content based subscription model by analyzing the specific message demand of health worker. Then, an efficient algorithm was proposed to match the message model and the subscription model according to the interests registered in the subscription list.2) We designed and implemented a medical message pushing system. The system includes several modules interacting each other. The SOA message publishing module collects messages from heterogeneous medical information systems. The subscription module provides users with user friendly subscription interface, through which the subscribers submit their demand for messages. The subscription module provides the management of subscriptions. The matching module provides effective query for the proper subscribers from the subscription list. The delivering module provides reliable message transportation service, and pushes the message accurately to terminal of subscribers. The displaying module provides a customizable message display.3) The system was implemented in the real clinical setting in a 3A hospital. Firstly,52 topics are summarized based on actual needs of health workers. Then a publishing server, a pushing server and a client are implemented to put the medical message pushing system into practice. So far, the message pushing system has been used in 50 departments and become an important tool for health workers in their routine work. The number of irrelevant messages has been decreased by 50.2% compared to previous system.The result shows that the proposed medical message pushing system based on pub/sub mechanism can express the specific demand of health workers accurately and push messages according to the subscription of health workers. The system effectively reduces irrelevant messages, providing an effective way to solve the problem of remind fatigue.
