Objectives: Based on the review of the literature, this paper reviews the policy development of the new rural cooperative medical system(NCMS) in Gansu province since 2003, describes the operation status of the new rural cooperative medical care in Gansu Province in 2009-2014, and evaluates the operating results of the new rural cooperative medical care in 76 counties(districts) by factor analysis method, to learn operation effect of the new rural cooperative medical system in Gansu Province as the whole, to promote the sustainable development of the new rural cooperative system, and the establishment of a scientific and effective performance evaluation system for reference.Methods: By literature review learning the research progress on development of implementation issues, the direction of reform, the evaluation of the effect and other aspects of the new rural cooperative medical system. And then using comparative analysis longitudinal describing the status of the new rural cooperative medical care, fund raising, fund expenditure, benefit situation of Gansu Province in 2009-2014, and evaluating the operating results by selected indicators in 76 counties(districts) by factor analysis method laterally. Using Excel2010 and SPSS21.0 for data entry and factor analysis.Results:1. Current status: The participation rate of the new rural cooperative medical care in Gansu has increased annually from 2009-2014. However, which below the national average. The financing level improved, the ability of fund collection was enhanced, which reached primary balance. But the gap between institutions was gradually widening, need to strengthen supervision.2. Effect evaluation: Four main factors were acquired to assess operation effect of the new rural cooperative medical system in Gansu Province by factor analysis, Factor 1: Financing factor, reflecting the financing ability of new rural cooperative. Score >0.00 as the standard, As shown in results, there were 45 counties(districts) scored as positive, 31 counties scored as negative, among them the county of Baiyin, Pingchuan, Subei and Akesai scored higher, whose scores all >1.00. Factor 2: Fund utilizes factor, reflecting the use of funds. As shown in results, there were 39 counties(districts) scored as positive, 37 counties scored as negative, among them the district of Ganzhou and Gangu scored higher, whose scores all >2.00. Factor 3: The actual compensation factor, mainly reflecting the NCMS actual compensation. there were 37 counties(districts) scored as positive, 39 counties scored as negative, Zhouqu county scored 2.07, 12 counties’ score between 1-2. Factor 4: Participation factor, reflecting the participation situation of new rural cooperative medical care. The score of Lintao county was highest. General evaluation: Compared with the comprehensive score of 76 counties in Gansu, the average score was 0.00, a total of 41 counties(districts) scores above the average score, 35 counties’ scores below the average. The score of Subei was highest, which was 1.42, the next was Baiyin, scored 1.11, score of Min county was lowest, which was-1.20.Conclusions:(1) The current status of new rural cooperative medical system in Gansu works well as the whole.(2) The main factors which affect the NCMS operation effect are financing ability, the use of funds, the actual compensation and participation situation of new rural cooperative medical care.(3) The NCMS operating results between counties(districts) in Gansu is imbalanced.Suggestions: To establish a stable and sustainable financing mechanisms of the NCMS, to strength the supervision and rational use of the NCMS funds; to improve the actual compensation level of the NCMS, and to integrate and coordinate the basic medical insurance system in urban and rural areas.
