Abstract/Summary:
Purpose: The 38 cases of cervical cancer patients eligible for in radical hysterectomy and pelvic lymph adenectomy, detect SCC-Ag level before and after operation, to analyze the relationship between SCC- Ag and clinical stage, differentiation degree change,pelvic lymph node metastasis. Evaluating the effect of the treatment, in order to guide clinical work.Methods: By chemiluminescence immunoassay detected levels of cervical cancer patients’ SCC-Ag which stage wereⅠB1-ⅡA2,there are 66 cervical cancer patients,and 38 patients with no obvious contraindication for radical hysterectomy and pelvic lymph adenectomy, post operativeagain to detect SCC-Ag levels.Statistical analysis using SPSS 20.0 software for processing, Measurement data do not accord with normal distribution, with a median of plus or minus quartile(M?Q), said the SCC-Ag level comparison before and after the treatment using paired sample rank and inspection; the spearman rank correlation analys is of SCC-Ag with the clinical stage, differentiation degree and lymph node metastasis; the relationship between the rate of compared by chi-square test, inspection level a =0. 05, P < 0. 05 for the difference was statistically significant. SCC-Ag> 1.5 ng/m L is supposed to positive, SCC- Ag <1.5 ng/ml is supposed to negative.Results: Adenocarcinoma and squamous cell carcinomas SCC-Ag positive difference was statistically significant(P <0.05), and squamous cell carcinomas SCC-Ag positive rate is higher; when before treatment SCC-Ag and lymph node metastasis into moderate positive correlation, and the rank correlation coefficient rs =0.636; SCC-Ag and clinical stage into a highly positive correlation, and the rank correlation coefficient rs = 0.956; SCC-Ag and differentiation degree before treatment no linear relationship. SCC- Ag level before surgery and lymph node metastasis of ROC curve drawing, SCC-Ag a value of 2.55 ng/ml, the area under the curve is 0.870, confidence interval: 0.775 to 0.965, has certain diagnostic accuracy, sensitivity and specificity, were 78.6%, 87.5%, false negative rate was 21.4%, the false positive rate was 12.5%, Youden index was 66.1; SCC-Ag level before and after the treatment for paired sample rank and inspection, difference was statistically significant(P = 0.00), after treatment SCC-Ag level is reduce.Conclusions:1. The SCC-Ag is a specific tumor marker for cervical squamous carcinoma, it can be used as auxiliary diagnos is index of cervical squamous carcinoma.2. Lymph node metastasis and clinical stage are the independent prognosis of cervical cancer, SCC-Ag just in clinical stages and lymph node metastasis in patients with predicted has clinical signif icance, to show that the SCC-Ag have reference value to judge prognosis of cervical cancer.3. The SCC-Ag level had significant difference before and after radical surgery, it can judge the treatment effect, and the surgery in patients with early cervical cancer is efficient.4. The SCC-Ag can be used as the judging indexes of clinical stages and lymph node metastasis,the clinical stages and lymph node metastasis are not only the important elements to decide treatments, but also influence the prognosis of patients. Lymph node metastasis is one of the indications of relapse after treatment, in short, levels of SCC-Ag can help to guiding clinical treatment decisions, and also can be used to judge the prognosis of patients with recurrence.
