Objectives: Retrospective analysis of the values of squamous cell carcinoma antigen(squamous cell carcinoma antigen.SCC Ag)in the preoperative diagnosis and postoperative prognosis of cervical cancer,discussion of its auxiliary significance and value in the cervical cancer clinical pathology,prognosis assessment and the neoadjuvant chemotherapy.Methods:This study chooses the cervical cancer patients about 154 cases and 40 the clinic healthy women(no gynecological diseases or other malignant tumors)who came to visit the Second Affiliated Hospital of Dalian Medical University Hospital from 2010 to 2016,the former as the experimental group and the latter as the control group(of which 114didn’t have the neoadjuvant chemotherapy or 40 cases had).The patients are eligibly selected according to the inclusion criteria and exclusion criteria.All in the experimental group were confirmed as cervical cancer patients by pathological diagnosis,and the relevant clinical data are available.SPSS 13.0 statistical software was used for x2 test,t test,single factor analysis of variance.P < 0.05 means a difference,that is,statistically significant.The ROC curves were plotted after comparing the serum SCC.Ag positive rates in cervical squamous cell carcinoma group,non-squamous carcinoma cervical cancer group and control group,and the optimal critical value of diagnosis was found.Compare the serum SCC.Ag expression levels of cervical cancer in various clinical stages,tissue differentiation,infiltration depth and lymph node metastasis;Determine the prognostic values of the total survival period according to the expression levels of serum SCC.Ag in the initial diagnosis.Compare the decrease degree of serum SCC.Ag in patients with neoadjuvant chemotherapy and evaluate the prognosis of patients.Consequences:1.Before surgical treatment,the expression levels of serum SCC.Ag in the cervical squamous cell carcinoma group,the non-squamous carcinoma group and the control group were successively observed as 63.41%,22.58% and 8.1%.The sensitivity and specificity of serum SCC.Ag expression for preoperative diagnosis of cervical squamous cell carcinoma were 63.41% and 91.90% respectively.2.The serum SCC.Ag expression levels and its clinical stage,tissue differentiation degree,infiltration depth and pelvic lymph node metastasis are obviously different in statistical significance(p < 0.05).Among them,there were 15 cases with Ia mean value of 2.67 + 0.51 ng/ml.There were 39 cases in Ib,with an average of 3.59 + 0.12ng/ml.There were 24 cases in IIa period,with an average of 5.83 + 0.94ng/ml.There were 5cases in IIb period,with an average of 7.31 + 1.39ng/ml.Histopathology showed a total of 14 patients with high differentiation,with an average of 2.45 + 0.87ng/ml.A total of31 cases were differentiated,with an average value of 3.89 + 1.26ng/ml;There were 38 cases with low differentiation,with an average of 5.34 + 1.41ng/ml.There were 28 cases with pelvic lymph node metastasis,the mean value was 6.22 + 0.72ng/ml;The mean value was 3.31 + 1.35ng/ml.3.The optimal threshold value of serum SCC.Ag before treatment was 2.41ng/ml for cervical cancer adjuvant diagnosis,and the optimal threshold for the prediction of cervical lymph node metastasis in early cervical cancer was 3.625ng/ml.4.The expression level of serum SCC.Ag detected in the initial diagnosis was divided into 2 groups,and the clinical value of the total survival time of patients was discussed by using the statistical kaplan-meier survival analysis.Among them,32 patients were still alive at the end of follow-up,and 8 cases were lost,with 43 cases in total..Analysis results show that the serum levels of SCC Ag positive expression of overall survival postoperatively in patients with cervical squamous carcinoma from a comparative differences statistically significant(Χ 2 = 45.412,P < 0.001).5.Comparing the expression levels of preoperative serum SCC.Ag in 40 patients undergoing neoadjuvant chemotherapy,it was found that when the serum concentration was reduced by 50%,the differences between the groups with the 5-year survival rate were obvious.After neoadjuvant chemotherapy,the expression levels of serum SCC.Ag decreased more,and the 5-year survival rate was higher,P<0.05 means a difference,that is,statistically significant.Conclusion:Serum SCC.Ag is a specific tumor marker for cervical cancer.The expression level of serum SCC.Ag increased with the improvement of clinical stage,the degree of histological differentiation and the degree of infiltration.On the other hand,the expression level of serology is also closely related to pelvic lymph node metastasis and has a certain relationship with the survival rate of patients.The level of serum SCC.Ag can be used to predict the metastasis of pelvic lymph nodes,and it is of certain significance to guide the treatment of postoperative adjuvant chemoradiotherapy.For patients with neoadjuvant chemotherapy for cervical cancer,the level of expression of preoperative serum SCC.Ag can predict the therapeutic effect,and it is of great significance for the development of postoperative monitoring.
