Objective To explore the feasibility and clinical significance of nipple-sparing mastectomy in surgical treatment of breast cancer.Methods From January 2008 to December 2015,clinical date of 88 patients suffered with breast cancer who underwent NSM plus SLND or ALND in Guangxi medical university affiliated tumor hospital was analyzed as a study group,and clinical date of 113 patients suffered with breast cancer who underwent mastectomy plus SLND or ALND during the same period was analyzed as a control group, some of them received breast reconstruction. To compared local recurrence rate, distant metastasis rate, disease free survival rate and overall survival rate between study group and control group. In addition,to observe the complications of necrosis at the nipple-areola complex (NAC) and to evaluate the reconstructed breast.Results The median follow-up time after surgery of study group and control group was 48 months and 51 months respectively. There were 2 cases of local recurrence at the NAC in study group and 2 cases of local recurrence in the chest wall in control group, the local recurrence rate was 2.3% and 1.8% respectively, there was no statistically significant difference (p=1.000). The distant metastasis rate was 6.8% and 6.2% respectively, there was no statistically significant difference (p=0.858). The 5-year disease-free survival rate was 87.8% and 92.5% respectively, there was no statistically significant difference(p=0.406). The 5-year survival rate was 96.9% and 99.0% respectively, there was no statistically significant difference(p=0.366). Nipple necrosis was not observed in study group. The satisfactory rate was 100%among 37 cases who received breast reconstruction in study group,.Conclusion Based on the strict selection criteria, NSM is safe and feasible in the surgical treatment of breast cancer. In contrast to mastectomy, local recurrence did not increase, moreover, NSM together with breast reconstruction could achieve satisfactory cosmetic results. NSM is worth popularization and application.
