| Objective:The study method of small sample randomized control had been used in this project. The control group is clomiphene. The clinical effect of the deficiency of the kidney and the cold womb infertility, which treated by the combination of abdominal acupuncture and moxibustion, are observed. This project aims to expand the effect of abdominal acupuncture and the acupuncture therapy applicable scope of infertility, and provide the rigorous scientific research material and the clinical new idea for the combination of abdominal acupuncture and moxibustion to treat the female infertility.Method:Most cases in the research are from outpatients and gynecology ward patients who visited the Traditional Chinese Medicine Hospital of Guangdong Province. In accordance with the standard the research set to select cases, 66 cases were equally randomized into treatment group(the group using abdominal acupuncture and moxibustion)and control group (the group using clomiphene), with 33cases each. After that, the treatment we investigated was conducted on a daily basis, One course of therapy consist of one menstruals cycle. In this study, the project must be completed three course of therapy. After 3 course of therapy, according to the index and comparison of The curative effect in prior treatment and post-treatment, we can make a quantization scoring in the end. SPSS 20.0 statistical software was used for dealing with all the data from the study.Result:In accordance with the standard the research set to select cases,66 cases who is Infertility due to kidney deficiency and ovulation failure were equally randomized into this project.33 cases in the treatment group(the group using abdominal acupuncture and moxibustion) and displacement(1 patient, because of treatment interruption),33 cases in control group(the group using clomiphene) and displacement(3 patient),62 cases are observed in reality.1. Comparison of the information between treatment group and control group before treatment:through statistic analysis and comparison, we can know that two groups’patients had no statistical significance in age, type of infertility, TCM syndrome type. The treatment group and control group are comparable.2. After 3 course of therapy, the index are observed and analyzed.(1) In the treatment’s situation of promoting ovulation,32 patients who has 95 menstrual cycle,85 ovulatory cycle and the ovulation rate is 86.32% in the treatment group.30 patients who has 89menstrual cycle,68 ovulatory cycle and the ovulation rate is 76.4% in the control group. through statistic analysis and comparison, we can know that two groups’ patients had statistical significance. and the treatment group is better than the control group.(2) Clinical rate of pregnancy. Clinical trail study about 32 cases of experimental group in which it had 18 pregnant cases, and rate of pregnancy was 56.25%. while the control group had 14 pregnant cases,46.67% pregnancy rate. According to the data, it was great difference between experimental group and control group, so experimental was better in cure rate.(3) Total rate of clinic curative effect. Total rate of clinic curative effect was divided by clinic effect standard of this trail, summing up the recover rate and effective rate. From the trail result,18 cases recovered,10 cases effected, and 4 cases non-effected so total rate was 87.5% in experimental group. Similarly, control group had 14 cases recovered,9 cases effected, and 6 cases non-effected and the total rate was 87.5% that showed no statistical significance in two groups (P>0.05).(4) Recovery of TCM syndrome. Total rate of TCM syndrome was divided by TCM syndrome cure effect in this trial, according to the degree of points change to estimate the treatment effect. It was comparable that TCM syndrome score of two groups was little different before the treatment. And after the treatment, TCM syndrome score of the experimental group was better than the former one (P<0.05), while the score of control group showed no difference between the former and latter (P>0.05), which meant the experimental group can improve the TCM syndrome and cure effect.(5) The change of BBT normalizing rate. Measuring the temperature of two groups, it found out group presented typical double phase 32 experimental cases was 78.13%, and the one of control was 76.67%. Statistically analyzing, the comparison of the two groups prior and after temperature was significantly different, but there was no different comparison among groups.(6) The change of endometrial thickness and follicular development prior and after the treatment. The endometrial thickness (Em) and follicular development(FD) before the treatment of two groups were no difference, but great difference in group self-contrast after the treatment. After the treatment, Em and FD had statistical significance in group-comparison (P<0.05). The Em data of experimental group was 9.83±1.01, while the control group was 7.94±2.13;the FD date of experimental was 20.46±2.56, while the control group was 18.59±3.04. The treatment group is superior to the control group in bettering the EM and FD, which indicates that while treating Infertility due to ovulation failure, the combination of abdominal acupuncture and moxibustion could obviously better the condition of the endometrium including the thicken the thin endometrium, and at the same time promote the growth of the ovarian follicle. This could cover the shortage of thinning endometrium of using clomiphene to promote ovulation.(7) Effect on the concentration of basic gonadal hormone in the blood. There’s no significant difference between the treatment group and the control group while considering the FSH, LH, E2 and T4 concentration in the blood before treatment. And it shows significant differences between while comparing the blood LH concentration before treatment and after treatment both in the treatment group and the control group (P<0.05), but no significant differences while comparing the other three blood gonadal hormone concentration before-treatment and after-treatment both in the treatment group and the control group. However, the betterment of the treatment group are generally superior to the control group’s, which indicates that the combination of abdominal acupuncture and moxibustion has an effect on the concentration of basic gonadal hormone in the blood. Conclusion:1. The combination of abdominal acupuncture and moxibustion is one of the effective treatments of Infertility due to ovulation failure. And it could adjust the patient’s endocrine and reproductive system, which helps to recover the ovary’s function, better the growth of the ovarian follicle, the normal ovulation and the thickness of the endometrium, as well as to improve the endometrial receptivity. All these good effects benefit to the nidation of the eggs.2. There are no significant differences between the treatment group and the control group while considering the general efficiency and BBT recovery rate. But the combination of abdominal acupuncture and moxibustion has a better effect than the treatment of using the clomiphene in promoting the ovulation rate, pregnant rate and bettering the Chinese medicine symptoms.3. Patients, who received the combination of abdominal acupuncture and moxibustion treatment, did not get untoward reaction during the treatment stage, and there is a lower threatened abortion rate in the treatment group than in the control group. Thus the combination of abdominal acupuncture and moxibustion is an safe and effective treatment of Infertility and it worth clinical promotion and further study. And these are what this study devote to do. |
