Clinical Study Of Treating Nervous Tinnitus By Needling Cooperating With Suspended Moxibustion

PurposeTinnitus is a subjective feeling of sound in the ear or in the brain of the patient, but there is no corresponding sound source or electric stimulation in the outside world. Western medicine treatment of nervous tinnitus stressed early diagnosis and early treatment, by using the drugs also with thysical therapy, by using the comprehensive treatment, but the clinical effect is poor. The Traditional Chinese Medicine with the "Yellow Emperor" as the representative of the ancient books on etiology, pathogenesis of tinnitus has elaborated in detail. After observation, there has been a long history of acupuncture and moxibustion in the treatment of tinnitus. The treatment to tinnitus is useful of Huoxue Tongluo, inducing resuscitation and rev closed. This paper research use the random control method to study the clinical effect of acupuncture corresponding with moxibustion in treating nervous tinnitus.MethodThis study adopts the method of randomized clinical trials with the nervous tinnitus unified diagnostic criteria and inclusion criteria. We selecte patient from June 2015-February 2016, in the department of acupuncture and moxibustion ENT in-patient and out-patient of the First Affiliated Hospital of Guangzhou University of TCM. A total of 60 patients were divided into 2 groups:treatment group with 30 cases and control group with 30 cases. Treatment methods:experimental study in the treatment group received acupuncture,the main points are Ermen, Tinggong, Tinghui, Yifeng, Hegu, Taichong, when the "qi" arrives, the total duration of needle retention for 30 minutes, during the time we use the moxibustion around the ear which is sick:Ermen, Tinggong, Tinghui, Yifeng for 20 minutes. Control group were treated with common acupuncture treatment without ather therapy,also for 30mins. Treatment:the treatment of the two groups was once every other day,3 times a week, the treatment period is 4 weeks. And after the completion of treatment we evaluate the tinnitus severity table, test of Hamilton Anxiety Scale, Hamilton Rating Scale for depression test, compared the data changes of the two groups before and after the treatment, organizing and collecting data for clinical research to establish a database, using SPSS 22.0 statistical software for statistical analysis of the data.ResultThere was no significant difference between the two groups in gender, age, tinnitus severity score, anxiety score and depression score before the treatment(P>0.05). According to Professor Liu Peng’s tinnitus severity assessment index and standard for evaluation, after the treatment, the total clinical effective rate in the treatment group was 93.33%; compared with the treatment group, the total clinical effective rate was 80%. Clinical testing by Ridit, the total efficiency of the two groups had significant difference (P< 0.05), the results suggested that the treatment group has higher clinical efficacy. Each TCM Type:comparing the curative effect of the treatment group, the curative effect is better than that of the control group, especially in treatment of deficiency syndrome of tinnitus, acupuncture combined with moxibustion effect is more significant. But in the treatment group, it shows efficacy to some extence in the treatment of fire disturbance, stagnation of phlegm fire type tinnitus, indicating that acupuncture combined with moxibustion on nervous tinnitus of various syndromes had certain curative efficacy. But due to the lack of sample size, the results had no obvious significance, we need to increase the quantity of samples to go on studying. The other scale results after the treatment, in the treatment group, the anxiety test score of patients before and after the treatment for compared by paired t test is t=15.24, P=0.0000**, showing a significant difference; in the control group, the anxiety test score comparison, by paired t test, t=15.53, P=0.0000**, suggesting that there is significant difference between the control group before and after the treatment; the anxiety test score between the two groups, with two independent samples t test, t=2.1342, P=0.0371*<0.05, suggesting that there is significant difference between the two groups. The depression test scores of the patients in the treatment group before and after treatment test by paired t-test, t=13.3031, P=0.0000**, suggesting significant difference; in the control group, by paired t=test, t=11.52, P=0.0000**, showing significant difference before and after the therapy; the result of the depression score of the two groups before and after the treatment tested by independent samples t test is that:t=3.9726, P=0.002*<0.05, suggesting that between the difference in the two groups of patients before and after the treatment with significant difference. (Note:* indicates the comparison between groups, P<0.05;## said the group before and after treatment were compared, P<0.05). The above data show that acupuncture combined with moxibustion and acupuncture on nerve tinnitus have a certain effect, but acupuncture combined with moxibustion for treatment of nervous tinnitus is better than the normal acupuncture.ConclusionAcupuncture combined with moxibustion and simple acupuncture therapy can improve clinical curative efficacy of patients with tinnitus in a certain extent, but the clinical curative effect of the former is superior to the latter. Treating nervous tinnitus by acupuncture combined with moxibustion, combined with the effect of acupuncture and moxibustion, and the therapy operation is simple, and the clinical effective quickly, the effective rate of the treatment is high, it worth to be applied to the clinical.
