The Research Of Health-related Quality Of Life And It’s Influence Factors In Patients With Type 2 Diabetes Mellitus Hospitalized

Objective:This study aims to research the quality of life(QOL) and it’s effect factors in type 2 diabetes patients hospitalized.The research results perhaps could provide evidence for the treatment and improvement of QOL.Methods:we use the scale QLICO-DM to survey 303 type 2 diabetes patients who were treated in the department of diabetes of the first affiliated hospital of kunming medical university during July 2015 to February 2016. And collect related information:gender、age、marital status、diet control、exercise、self-monitoring of blood glucose(SMBG)、BMI、vision loss situation、diabetic peripheral neuropathy (DPN)situation、the occurrence of hypoglycaemia; Then detect blood sugar levels、 lipid levels、HbAlc levels and so on. We use t-test, analysis of variance, Chi-Square tests and multiple linear regression analysis to process the measurement data. Finally,we discuss the effect factors of quality of life in patients with type 2 diabetes mellitus.Results:(1) A total of 303 patients were involved(men197,women106). The score of QLICO-DM was 155.91±13.973 (x±s). We use one-sample-t-test to compare the patients’ score and the full score of the scale,and there was statistical significance (P<0.0001).(2) The following factors were not associated with the quality of life in diabetes patients:gender、age、marital status、household register (village or city)、nation、level of education, newly diagnosed diabetes or not、 smoking、drinking、BMI、waistline、diet control、self-monitoring of blood glucose(SMBG)、lipid levels、islet cell function、glycemic control situation、 therapeutic method (no treatment、lifestyle intervention、oral medication、insulin therapy)、frequency of insulin injections.There was no statistical significance (P> 0.05).(3) The patients whose course of disease≥5 year have lower quality of life, there was statistical significance (P=0.05). The patients who exercise more than 150min/week have better quality of life, there was statistical significance (P=0.018). Numbness of limb(DPN) was associated with lower QOL, there was statistical significance (P<0.0001). Vision loss was associated with lower QOL, there was statistical significance (P<0.0001). Diabetic kidney disease was associated with lower QOL, there was statistical significance (P<0.0001). Hypoglycaemia was associated with lower quality of life, there was statistical significance (P=0.013).(4) The above-mentioned 6 factors were involved in the multiple linear regression analysis.The resault showed that the effect sequence of the factors to the quality of life is:numbness of limb(DPN) (P<0.0001)、vision loss (P=0.002)、diabetic kidney disease (P=0.008)、exercise time (P=0.057)、hypoglycaemia (P=0.126)、the course of disease(P=0.876).Conclusions:The quality of life of type 2 diabetes mellitus hospitalized decreases universally. The following factors are associated with lower QOL:numbness of limb(DPN)、vision loss situation、diabetic kidney disease、hypoglycaemia、exercise time、the course of disease.The effect sequence of the factors to the quality of life is: numbness of limb(DPN)、vision loss、diabetic kidney disease、exercise time、 hypoglycaemia、the course of diseas. In the treatment and management of diabetes patients,we should pay more attention to the QOL of the patients who have longer course of disease. We should prevent and treat diabetes complications, increase in exercise time, avoid hypoglycaemia to improve QOL of diabetes patients.
