Objectives To evaluate the prevalence, associated risk factors and impact on generic and condition-specific health related quality of life (HRQL) of overactive bladder (OAB) in patients with type 2 diabetes mellitus (DM).Methods A total of 868 patients with type 2 DM were included by convenience sampling in endocrine outpatient in a general hospital in Jinan. We used Overactive Bladder Symptom Score (OABSS) and Indevus Urgency Severity Scale to evaluate the symptoms of OAB. We used SF-12, Overactive Bladder Questionnaire Short Form (OAB-q SF) and Patient Perception of Bladder Condition (PPBC) to estimate the impact of OAB on HRQL. Data analysis was conducted with SPSS 17.0.Results1. The prevalence of OAB, OABwet and OABdry in diabetes mellitus patients were 13.5%,7.6% and 5.9%, respectively. The prevalence of mild, moderate and severe OAB was 29.1%,65.8% and 5.1%, respectively. The prevalence of mild, moderate and severe urgency was 27.1%,35.3% and 37.6%, respectively.2. Multivariate analysis showed that age, gender, HbAlc, retinopathy, diabetes treatment, tranquilizer or analgesic, obesity and toileting behavors were significantly associated with OAB.3. The mean of bother subscale and HRQL subscales of OAB-q SF were (33.84±27.38) and (72.07±21.14). The mean of sleep subscale (64.32±31.70) was the lowest among HRQLsubscales. Of the patients who had OAB,61.9% had little or some minor problems,15.2% had moderate problems and 22.9% had severe problems.4. SF-12 scores for all domains were lower for DM patients who had OAB. Differences between patients with OAB and OAB-negative were statistically significant for all SF-12 domains after adjusting for covariates including age, gender, diabetes-related variables and comorbidities.Conclusions1. The prevalence of OAB in mainland China is lower than that of previous studies conducted in patients with type 2 diabetes mellitus and higher than that of previous studies conducted in general population in mainland China. The OAB symptoms in diabetes mellitus patients are more severe compared with that of previous studies conducted in general population.2. Older age, male, higher HbAlc level, retinopathy, tranquilizer or analgesic, obesity and poor toileting behavors are independent risk factors for OAB in diabetes mellitus patients. Treatment with oral antidiabetic drugs or antidiabetic drugs combined with insulin are protective factors for OAB compared with diet treatment.3. The symptoms of OAB have a detrimental effect on HRQL including physical health and.mental health in DM patients.
