Objective:To investigate registered clinical nurses’ characteristics of nursing competence and job satisfaction. To explore the effects of nursing clinical ladder program on nursing competence and job satisfaction of registered clinical nurses. In order to provide a theoretical basis for stimulating initiative of nursing staff and improving the level of nursing management.Method:This study used quasi-experimental method. Questionnaire was employed in this study. Before implementing clinical ladder program, an investigation was conducted in registered clinical nurses in a major hospital with Nursing Competence Scale (NCS) worked out by Li Xue-mei and Nursing Job Satisfaction Scale (NJSS) worked out by Chen Mei-fang. After the implementation of clinical ladder program of one year, used the same two scales on the same nurses. Statistics on the data which obtained in the investigation were conducted by use of SPSS software for windows version19.0.Results:1. The total score of the NCS is 74.38±9.88. Among them, critical patient care score>management score>interpersonal communication score>professional development score, health education score, nursing process applying score(F=32,428,P<0.01). There are significant differences of the nursing competence of nurses among age, experience, education, marital status, department, night job, professional position, work position, teaching activities, family support, task significance, perceived future of nursing, will, clinical ladder(P<0.05).2. The total score of the NJSS is 79.9210.37. Among them, management satisfaction score>income satisfaction score, colleague satisfaction score>promotion satisfaction score, society support satisfaction score (F=32.428,P<0.01). There are significant differences of the nursing job satisfaction of nurses among age, experience, education, night job, professional position, work position, family support, task significance, perceived future of nursing, will, clinical ladder(P<0.05).3. After implementing clinical ladder program, the total score and the score in the dimensions of nursing competence and nursing job satisfaction of nurses are significantly improved(P<0.05).4. After implementing clinical ladder program, the total score of nursing competence and the score in nursing process applying, health education, interpersonal communication of level Nl nurses are significantly improved(P<0.05); the score in critical patient care, interpersonal communication of level N2 nurses are significantly improved(P<0.05); the total score of nursing competence and the score in nursing process applying, interpersonal communication, management, professional development of level N3 nurses are significantly improved(P<0.05); the total score of nursing competence and the score in interpersonal communication, management, professional development of level N4 nurses are significantly improved(P<0.05).5. After implementing clinical ladder program, the total score of the nursing job satisfaction and the score in colleague satisfaction, management satisfaction, income satisfaction of level N1 nurses are significantly improved(P<0.05); the score in colleague satisfaction, management satisfaction of level N2 nurses are significantly improved(P<0.05); the total score of the nursing job satisfaction and the score in the dimensions of the nursing job satisfaction of level N3,N4 nurses are significantly improved(P<0.05).Conclusions:1. The level of nursing competence of registered clinical nurses in a major hospital shows a medium level which needs to be further enhanced.2. The level of nursing job satisfaction of registered clinical nurses in a major hospital shows a medium level. Nursing managers should pay attention to the demand of registered clinical nurses for society support and promotion3. The clinical ladder program can effectively improve the nursing competence and job satisfaction of nurses with different ladders.
