ObjectiveRetrospectively evaluate and analyses the clinical and radiographic outcomes of patients undergoing the modified McLaughlin procedure for chronic unreduced posterior dislocation of the shoulder, in order to further improve clinical cognitions to the posterior dislocation and provide basis for the treatment.MethodsA retrospective review of four patients who underwent this procedure in the 2nd affiliated hospital of Zhejiang University from September 2014 to February 2016 was carried out. All of the four patients were male. The average age was (39.5±2.4) years (range, 38-42 years). Three of them resulted from trauma and one from epileptic seizure. One patient had left arm involved while the other three got right side involved. The average time from initial injury to surgery was (23.5±23.6) weeks (range,3-53 weeks). These procedures were all performed by the senior orthopaedic surgeons in this hospital. Clinical functions and radiological results of the four patients were evaluated preoperatively and postoperatively. The outcomes were assessed in terms of function, pain, and patient satisfaction with the help of the Constant-Murley score at the last follow-up. After the data was collected, tabulate it for statistical analysis.ResultsThe mean follow-up period was (7.5±7) months (range,1-18 months). No complications were noted and no recurrences were seen. Mean range of shoulder motion was (138.8°±33.0°) of flexion (forward elevation:range,95°-175°), (136.3°±16.5) of abduction (range,120°-155°), (46.3°±11.1°) of external rotation (range,30°-55°), and (29.8°±6.9°) of internal rotation. For the four cases, the average preoperative Constant score was 37.8±7.4, but at last follow-up, the average Constant score achieved was 81.8±7.4, which was significantly improved (P<0.05).ConclusionThe modified McLaughlin procedure is a reliable operation and produces satisfactory results for chronic posterior dislocation of the shoulder.
