Objective:To investigate the clinical effect of posterior Monteggia fracture in adults with or without dislocation of the humeral ulnar joint.Methods:This study collected clinical data of adult patients with posterior Monteggia fracture treated by open reduction and internal fixation in the Second Hospital of ShanXi medical University,described the treatment and functional results of 5 adult patients with posterior Monteggia fracture combined with posterior dislocation of the humeral ulnar joint,compared with 11 cases without posterior dislocation of the humeral ulnar joint,and analyzed the difference in prognosis.Results:Among the 5 patients with humeral ulnar joint instability,there were 4 males and 1 female,with an average age of 49 years(28-85 years).The injury mechanism was 4 fall injuries and 1 traffic accident injury.The elbow function of 5 patients after operation: the average flexion was 98 °(80 °-120 °)the average loss of elbow extension was 18 °(10 °-25 °)the average pronation was 58 °(50 °-70 °)and the average pronation was 66 °(50 °-90 °).The standard DASH score was 43.2 on average,and the Broberg Morrey functional index score was 73.2 on average.the results were excellent in one,good in two and poor in two.According to the Broberg Morrey classification,the imaging evaluation included 1 case of grade 0,1 case of grade 1,1 case of grade 2,and 2 cases of grade 3;There were 6 males and 5 females without dislocation of the humeral ulnar joint.The elbow function of 11 patients after operation: average flexion 117 °(90 °-140 °);average loss of elbow extension 5 °(0 °-20 °);average pronation 63 °(50 °-80 °);average pronation 61 °(30 °-90 °).The DASH score was 20.6 on average,and the Broberg Morrey functional index score was 82.7 on average.There were 3 excellent,6 good,and 2 poor.According to the Broberg Morrey classification,there were 2 cases in grade 0,4 in grade 1,3 in grade 2,and 2 in grade 3;Conclusion:The range of flexion and extension of elbow joint in patients with posterior Monteggia fracture combined with posterior dislocation of humeral and ulnar joint was significantly lower than that patients without dislocation of humeral and ulnar joint.DASH score,Broberg Morrey index score and excellent and good rate were also lower,and the incidence of postoperative complications was higher?...
