Objective:Through observation and comparison of the index of two therapeutic methods which are designed for patients with symptoms of proximal humeral fracture together with shoulder dislocation, such as fracture reduction, function of shoulder joint and fracture healing time, study the clinical effects of over-shoulder traction methods in terms of healing shoulder dislocation as well as proximal humeral fractures. It provides the reference in field of non-surgical treatment of proximal humeral fractures together with shoulder dislocation.Methods:Selecting patients who saw doctors for proximal humeral fractures together with shoulder dislocation clinic or in ward in Suzhou Hospital of Traditional Chinese Medicine during the period from August,2014 to August,2015 and later divided 23 patients into external fixation with reset plywood group (experimental group) and internal fixation with open reduction group (control group) according to therapeutic methods. After six-month post-operation survey, observing fracture reduction, function of shoulder joint and fracture healing time. In addition, statistical analysis is utilized to compare curative effects between groups.Results:1. After two-week treatment, comparing the reduction situation of fracture and can be find that excellent and good rates of experimental group and control group reach 41.67%, 81.81%, respectively. Examining the results of two groups by Mann-Whitney test, it can be concluded that P<0.05, which has statistical significance.2. After six-month treatment, the pain scores of experimental group and control group are 10.16±1.34 and 9.00±0.77, respectively. Examining the results of two groups by independent sample T, it can be concluded that P<0.05, which has statistical significance.3. In terms of Daily Function Activity Score, after six-month treatment, experimental group reaches 12.75±1.06 while control group reaches 11.64±0.92, Examination of the results of two groups by independent sample T reveals the statistical significance (P<0.05).4. In terms of Muscle Strength Score, after six-month treatment, experimental group reaches 17.17±0.94 while control group reaches 18.82±0.75, Examination of the results of two groups by independent sample T reveals the statistical significance (P<0.05).5. The clinical fracture healing time of experimental group is 5.67±0.83 weeks while that of control group is 6.59±0.94 weeks. After the examination of the results of two groups through independent sample T, it can be seen that statistical significance is obvious (P=0.03<0.05).Conclusion:Obviously, in terms of proximal humeral fracture together with shoulder dislocation, over-shoulder traction methods, splint external fixation and functional exercise for treatment should be utilized and they are better than internal fixation with open reduction group (control group) in aspects of fracture healing time and improvement of daily life during six-month period. It deserves clinically extensive utilization.
