Abstract/Summary:
BackgroundCervical cancer is one of the most common malignant tumors in women. In the world, the new cases of cervical cancer every year 600 thousand, the death of about 300 thousand. China new cases each year about 160 thousand, about 80 thousand people died, the main reason is the lack of prevention of early diagnosis and treatment of early diagnosis and treatment program. Therefore, it is very important to study the epidemiological data of cervical cancer and its risk factors. The occurrence and development of cervical cancer is a multi factor and common biological process, almost all (99.7%) of cervical squamous cell carcinoma is caused by human papilloma virus (HPV) infection. HPV is a group of double stranded DNA virus, belongs to papillary virus, is a non enveloped virus is composed of DNA core protein and capsid. HPVDNA restriction enzyme map is different, the protein of the nuclear shell is different, which determines the HPV has a variety of typing. At present, more than 200 kinds of HPV subtypes have been found, which are divided into two parts:skin type HPV and genital epithelial type HPV. The main cause cause risk, by which about 50 kinds of subtypes can infect the genital system of HPV in inducing reproductive tract malignant tumor in different divided into low-risk and high-risk groups:HPV6,11,40,42,43,44,54,61,70,72,81, cp6108 low-risk type condyloma and low grade of cervical squamous intraepithelial lesions and benign lesions of the main etiology; HPV 16,18,31,33,35,39,45,51,52,56,58,59, 68,73 and 82 high-risk women cervical cancer and high grade cervical intraepithelial neoplasia.15 high-risk types of HPV and cervical cancer and cervical injury have a direct relationship, which HPV 16,18,31 and 45 subtypes and cervical squamous cell carcinoma is most closely related, and HPV 16,18 and 45 subtypes of the most easily lead to cervical adenocarcinoma. High-risk types of HPV, the typical characteristics is induced and terminal differentiation stage cells in DNA synthesis, and by the E6 and E7 oncoprotein to achieve; E6 and E7 by changing the terminally differentiated epithelial cells in the body, destruction of the negative regulation of the cell cycle, induce cells to enter the S phase and infect cells, and the expression of E6 and E7 oncoprotein infected cells to bypass the normal cell cycle checkpoint, resulting in genetic damage, and ultimately the development of tumor.Sexual life is active in 50% women will get more than one HPV infection, but more than a sex, and sustained high risk of HPV infection is the occurrence of cervical lesions of the necessary conditions. And the cervical high risk HPV infection in the presence of multiple risk factors, including early marriage and childbearing, prolific, sexual disorders, multiple sexual partners, hypoimmunity, smoking, taking birth control pills, and different factors in different regions and different populations the results are not the same. Therefore, prevention and control of cervical HPV infection can be used for primary prevention of cervical cancer.ObjectiveTo explore the risk factors of cervical high risk HPV infection, and to provide reference for the reduction and removal of high-risk HPV infection.Methods1 research objectsCollect 2014 January to 2015 10 months of Southern Medical University; the Third Hospital Affiliated to treatment for cervical lesions screening in 1471 patients, were conscious, normal intelligence, voluntarily cooperate with the investigation, completed survey scale of all content. According to the principle of voluntary choice of cases, the sexual life history, non pregnant state, with an intact uterus, by PCR fluorescence probe method in detection of the high risk HPV-DNA PCR positive 650 cases as study group; And 821 cases of high risk type HPV-DNA fluorescent negative were taken as control group.2 data acquisitionBased on the relevant literature at home and abroad and combined with expert opinion, the risk factors of high risk type HPV infection were designed. The investigation content includes:name, age, occupation, nationality, residence, educational level, income level, smoking or passive smoking history, drinking history, sexual partner number, contraception, sexual health habits, first sexual life age, first delivery of age, pregnancy, menopause, gynecological diseases, and her husband disease history, cervical cancer family history factors. After obtaining the informed consent of the investigation, the medical staff who were trained by the training to do the high risk HPV test directly asked the survey, fill in a unified questionnaire.3 experimental methodsWere detected by kit Chaozhou Kaipu biochemistry Co. ltd.. Detection of 13 high-risk HPV infection. Standard:more than 500IU/ml positive. Instrument:Roche LightCycler fluorescence quantitative PCR instrument.4 statistical analysisAfter verifying and sorting the questionnaire, the single factor and multi factor non conditional Logistic regression analysis were performed by using the SPSS 13.0 statistical software.ResultThe high risk HPV infection of cervix is affected by many factors. The total of 1471 cases of cervical lesions, aged between 18-70 years old, the maximum 70 years old, the smallest 18 years old, the age span of the survey object. Case group of 650 cases, the average age of 40.45±9.24 years old, the median age of 40 years:821 cases of the control group, the average age of 36.85±9.13 years old, the median age of 37 years.1 single factor non conditional Logistic regression analysis1.1 The two groups in age distribution have significant difference (P< 0.01), and distribution in the nationality, place of origin, had no significant difference.1.2 the relationship between occupational, educational level, income level and cervical high risk HPV infectionTwo groups of people in the cultural level, income levels and high risk HPV infection difference was statistically significant (<0.01 P), and there was no significant difference in occupation.1.3 the relationship between smoking, alcohol drinking and HPV infection of cervical high risk typesmoking history, passive smoking history, drinking history were the risk factors of cervical high risk HPV infection, and the relationship between smoking history and cervical high-risk HPV infection was statistically significant (P< 0.01).1.4 Relationship between sexual partner number, sexual health habit and HPV virus infection in high-risk cervicalfor the first time sexual life age is prone to cause cervical high-risk human papillomavirus infection and the relationship with cervical high risk HPV infection has statistical significance (P< 0.01). Two sides of sexual life before the two sides of the habit, a single sex partner is a high risk of cervical HPV infection of the protective factor, and its relationship with cervical high-risk HPV infection is statistically significant (P< 0.01).1.5 the relationship between contraceptive measures and cervical high risk HPV infectionthe use of contraceptives and intrauterine device, sterilization, prone to cause cervical high-risk HPV infection and statistical significance (P< 0.01). Long term use of condoms can reduce the risk of cervical HPV infection and its relationship is statistically significant (P< 0.01).1.6 the relationship between reproductive history and cervical high risk HPV infectionthe first childbirth age, pregnancy times cause cervical high-risk HPV, and there was statistical significance (P< 0.01). The risk factors of cervical high risk HPV infection were the small age of primary delivery, frequent abortion and medical abortion.1.7 The relationship between personal history and HPV infection in cervical high risk typethe history of cervical injury was a risk factor for cervical high risk HPV infection, and the relationship between menopause and cervical high risk HPV infection was statistically significant (P< 0.01). cervical cancer family history of infection of cervical high-risk HPV was statistically significant (P< 0.01):history of puerperal infection, uterine fibroids history, ovarian cancer history, family history of cervical cancer is the risk factors of high risk HPV.1.8 the relationship between vaginal infection and cervical high risk HPV infectionvaginal odor is the risk factors of cervical high risk HPV infection, the difference between the two groups was statistically significant (P< 0.01); abnormal vaginal discharge, vaginal odor is the risk factors of cervical high risk HPV infection.1.9 the relationship between husband’s disease history and HPV infection of cervical high risk typethere was a significant difference in the history of nasopharyngeal carcinoma and the high risk of infection with HPV, and the difference was statistically significant (P< 0.01). The history of prostate disease and nasopharyngeal carcinoma is the risk factor of cervical high risk HPV infection.2 multi factor non conditional Logistic regression analysis2.1 the relationship between social factors and cervical high risk HPV infectionAge, education, income levels and other factors in the comparison of the two groups have significant difference and the difference was statistically significant (P< 0.05). Thus it can be seen that these social factors and cervical high risk HPV infection is closely related.2.2 the relationship between behavior habit and HPV infection of cervical high risk typeDrinking history, did not take birth control pills, condoms time, sexual partner number, before cleaning habits, first age at delivery, delivery times, abortion abortion times factors and cervical high risk HPV infection of the relationship was statistically significant (P< 0.05).2.3 The relationship between personal history and gynecological history and HPV infection of cervical high risk typeCervical surgery injury history, sterilization operation history, cervical cancer family history factors in the comparison of the two groups of difference was statistically significant (P< 0.05) and cervical surgery injury history, sterilization operation history, cervical cancer family history are risk factors for cervical high risk HPV infection.2.4 Husband’s disease and the relationship between human papillomavirus infection and cervical high risk HPV infectionThere was no significant difference in the two groups of patients with nasopharyngeal carcinoma (NPC) and the difference was statistically significant (P< 0.05). The risk factors of nasopharyngeal carcinoma (NPC) were the risk factors for HPV infection. Conclusion1 low education, low income and high-risk HPV infection associated.2 smoking, drinking and other unhealthy habits, unhealthy sexual behavior and sexual health habits can increase the risk of cervical high risk HPV infection. The use of condoms is a protective factor for cervical high risk HPV infection.3 the age of first sexual life is small, the initial delivery age is small is the risk factors of cervical high risk HPV infection. Women with cervical HPV infection in a dose-response relationship.4 husband has a history of nasopharyngeal cancer, family history of cervical cancer is a high-risk HPV infection risk factors...
