Current Research On Vulnerability In A General Hospital Disaster Based On The Kaiser Model

BackgroundHospital disaster vulnerability means the ability to cope with these pressures and disturbance which the hospital may suffer losses from natural or social environment The vulnerability analysis of Hospital disaster is a systematic method to identify the potential risks hidden in the hospital which can find risk events, and raised the question of targeted recommendations.To carry out the disaster vulnerability analysis can improve the accuracy of hospital emergency decision-making, scientific, and improve the whole emergency ability of the hospital. In recent years, a variety of public events such as sudden natural disasters and terrorist activities have occurred frequently, and when the disaster occurred, the hospital is not only a fragile hazard bearing body but also the most indispensable infrastructure. So, how to survey the hospital disaster vulnerability analysis, early identification of disaster risk, and targeted to take scientific and effective prevention and response, reduce disaster of hospital operation and the impact of medical work, has become an important task to the current hospital emergency work and serious challenges,and it’s also an important basic work of hospital emergency management implementation of "prevention first, pass forward".ObjectiveTo sequence on the various potential risk to the hospital, to determine how to deal with the risk events t and to analyze the reasons for its formation and its impact on the hospital may have caused, thus to strengthen risk management, and to minimize the hospital disaster vulnerability.MethodsBased on the Kaiser Model, and used the questionnaire, to analyze the hospital disaster vulnerability. CNKI, Wangfang, VIP database and PubMed were used to search the "Disaster vulnerability analysis, vulnerability analysis,emergency, hospital emergency" and other related journal articles,The relevant documents and reports were searchd abouthospital emergency response capacity buildingand hospital vulnerability analysis, and these reports were used to determinethe four categoryes of 32 kinds of risk as a key event the study such as natural disasters, accidents, disasters, public health emergencies, and unexpected social security incidents. And grading standards evaluation factor were established from the occurrence of possible,severity (injury-prevention),Select comprehensive index method for disaster vulnerability survey, according to the results of the qualifying event risk, the key risk event response correlation analysis, and concluded that to propose the control strategy and recommendations.ResultsThe hazard of the hospital were the medical dispute (45.79%), the risk of traffic accidents (40.93%), fire risk (40.24%),water pollution risk (38.19%) and the risk of nosocomial infection (35.84%). The minimal risk was tornado hurricane (19.75%).Among the respondents, disputes under senior personnel thought hospital violence impact damage to property and service more severe accounted for 80% and 84%, respectively,55% of clinical and nursing professionals believed that the preparedness of the hospital medical disputes violence was low.71% of medical stuffs of medical department thought the reaction of cope with the violence was low.60% of middle-level cadres that the degree of a traffic accident on the service was higher.49% of non-clinical staff believed that the impact of the fire service was higher.48% of middle-level thought that the incident caused a large fire service impact was higher.73% of women and nurses believed that the impact of water pollution damage caused was higher.21% of the respondents whose working years were from 21 to 30 years thought hospital had a lower risk of hospital infection, 71% of the respondents whose working years were longer than 31 years thought hospital had a higher risk of hospital infection. The respondents of the department functions of hospital thought the infection caused by injury, property damage and the service were higher,accounted for 80%、77% and 87% respectively. The respondents of the department personnel thought the preparedness of infection of medical and external reaction were lower, accounted for 57%、86% respectively. 53% of non-clinicalandhospital care professionals thought that the emergency preparedness of hospital infection was lower.Conclusions and suggestionsIt is important to prevent violence in risk events mainly about medical disputes, traffic accidents, water pollution, fires and hospital infection in the hospital. The results of the vulnerability assessment related to the investigation’s department, position, length of service and professional factors, we need to pay attention to the choice of respondents when met different risk events. Response to risk events, the hospital can strengthen the construction of mechanism and system, set up early warning system, targeted to develop or improve the corresponding plan, organize training and drills, the reserves of the corresponding emergency supplies, and reduce the impact of disasters on hospital operation and health care work, to minimize the hospital disaster vulnerability.This research attempt to analyze the hospital vulnerability, and provide a reference method for medical institutions or research institutions system in hospital disaster survey.
