| Objective:To investigate the situation of yin-yang deficiency in the patients of senile skin pruritus and to observe clinical efficacy of Wenyangtongluo Decoction on the yang deficiency type of senile skin pruritus,also detecting the content changes of substance P and IgE.Method:Study 1:During the cross-sectional investigation, patients were collected from Guang’anmen Hospital and all of them in accordance with the inclusive criteria. Informations of symptoms and four diagnostic information of TCM were collected by filling in the senile skin pruritus cross-sectional investigation information collection table.After establishing a database,sum up the distribution law of syndromes by statistical analysis.Study 2:In this clinical randomized controlled trial,60 Patients who met the inclusive criteria were randomly divided into two groups, the treatment group and the control group, each of the group have 30 cases. Patients in the treatment group were treated with Wenyangtongluo Decoction; the control group with dermatosis blood poisonous bolus,both of two groups of Silicone emulsion cream for external use.After a total treatment duration of 4 weeks,pruritus signs and desiccation symptoms were evaluated as therapeutic effect.At the same time detecting the content changes of substance P and IgE between the patients and healthy senile people by ELISA testing.Results:Study 1:A total of 100 patients (57 men and 43 women) were included,their average age was 66.16 years old and course of the disease was 49.93 months of which the course was longer than 1year take 72 percentage. The percentage rates of patients’ Yin-Yang syndrome classification are as follows:yin deficiency type 11,yang deficiency type 66,yin and yang deficiency 23;of the 100 patients,19 cases with single symptom,81 cases have accompanied symptoms.The percentage rates of 81 patients’accompanied symptom types are as follows.sputum turbid stasis obstructing collaterals syndrome 56.8,retention of damp-heat syndrome 32.1, blood stasis vessel obstruction syndrome 11.1%.In this research the most frequent five symptoms are as followsrphysically and mentally fatigued(81),extreme chilliness(79),pale fat tongue(77),frequent urination at night(76),insomnia(76).Study 2:A total of 60 patients were enrolled with 30 cases in each group.Two groups of patients were comparable at gender, age,duration and symptom integral before the treatment(P>0.05). The total effective rate of treatment group 90%, control group, the total efficiency of 50%. The total effective rate in treatment group was significantly higher than the control group, statistically significant differences (P<0.05). Two groups of patients after treatment of patients with pruritus drying total scores were significantly improved, with statistical significance (P< 0.05). Compared with the control group, the treatment group has improved significantly at itch degree,itch location and itch frequency after treatment, with statistical significance (P<0.05); the treatment group after treatment of patients with SP and IgE improvement, without statistical significance (P>0.05).The complete therapy was well tolerated and was not associated with an excess of any side effects.Conclusion:1.The senile skin pruritus patients with yang deficiency syndrome take an 66 percentage,mnuch more than the single yin deficiency syndrome.It happens more frequency in the patients with yang deficiency symptoms like fatigue, chilliness and frequent urination at night.We should pay attention on this situation.2.Wenyangtongluo recipe is effective on the yang deficiency type of senile skin pruritus.It can significantly improve the skin itching and drying situation.There is no relevant between the changes of substance P and IgE and its mechanism.This therapy deserves further clinical applications. |
