The Clinical Applying Research On Local Fumigation With Chinese Medicine Combined With Kerotherapy For The Third Lumbar Verterae Transverse Process Syndrome Patients

Objective1 To explore the application effect of patients treated by local fumigation with Chinese medicine, local fumigation with Chinese medicine combining with wax therapy for the third lumbar verterae transverse process syndrome patients which aimed at providing reference and data for clinical nursing interventions.2 To explore patients with real experience of traditional Chinese medicine nursing technique,such as the acceptance, recognition, requirements and obedience, so that we can improve the clinical application of traditional Chinese medicine nursing technology futuer.MethodsThis research combined the quantity with quality as research methods. ①Quantitative study:Taken the convenience sampling as a method, patients who accordded with the inclusive criteria of the third lumbar verterae transverse process syndrome were from orthopedics and traumatology in a third-level hospital of traditional Chinese medicine in Harbin and as objects of the research. The control group left 32 cases, were only treated by hospital conventional care and treatment; the test Group 1 left 33 cases, were treated by local fumigation with Chinese medicine on the basis of the control group; the test Group 2 left 33 cases, were treated by kerotherapy on the basis of test groupl. To evaluate Japanese Orthopedic Association Scores, Oswestry Disability Index, life-BREF before and after the intervention. All of datum were analyzed by SPSS 19.0 and there would be statistically significant difference when P< 0.05 (A standard for inspecting of alpha= 0.05). (2) Qualitative study:Taken purposive sampling as a method, the representative patients were chosen from the test group 2 as objects of the research 11 cases patients would be interviewed when the datum saturated, which were transcribed, analysized according to seven steps of analysis in Colaizzi phenomenology, and extracted four themes.Results1 Before the intervention, comparing each dimension and total scores of JOA, scores of ODI, each dimension and total scores of quality of life between the three groups of patients with general information, there ere were not statistically significant differences(P>0.05).2 Before and after the intervention, respectively comparing each dimension and total scores of JOA, scores of ODI, each dimension and total scores of quality of life in the three groups, there were statistically significant differences (P< 0.05).3 After the intervention, there were statistically significant differences between the three groups in each dimension and the total scores of JOA, each score of ODI (P<0.05). There were statistically significant differences between the three groups in the field of physical and social relations in quality of life (QOL)(P<0.05); but There were not statistically significant differences in the field of psychology and environment(P>0.05). There were not statistically significant differences in total scores of quality of life between control group and test group 1,between test group 1 and test group 2 (P>0.05).4 Qualitative study induced to four themes:(1) Cognitive experience of disease:deficiency of cognition of disease; The disease has a great impact on patients.(2) Deficiency of cognition of traditional Chinese medicine nursing technology.(3) Accept the experience of the attitude and behavior to Chinese medicine nursing technique:before and after Traditional Chinese medicine nursing, acceptable attitude of patients were a big difference; and the acceptance and obedience were poor;(4) The experience of service demands for Chinese medicine nursing technique:Good service attitude and enthusiasm for work; Strengthen the psychological intervention and guidance; Build a scaled and humanized service environment.Conclusion1 Local fumigation with Chinese medicine, local fumigation with Chinese medicine and combined with kerotherapy Kerotherapy can improve the symptoms and signs, also improve the quality of life.2 improvement of being treated by local fumigation with Chinese medicine and combined with kerotherapy was better than patients with local fumigation with Chinese medicine.3 In the clinical work, we should improve the cognition of the patients to diseases, and promote the nursing technology of traditional Chinese medicine actively, so that social identification and constantly the management of Chinese medicine nursing technology can be improved.
