Objective:To collect the clinical data of patients with cerebral small vessel disease(CSVD), in order to investigate risk and independent risk factors of cognitive and emotional impairments caused by CSVD.Methods:In this study, we studied the clinical data of 93 patients with CSVD in the eastern division of the First Hospital of Jilin University’s neurology ward from January 2015 to December 2015.According to the scores of Mo CA, the patients were divided into cognitive impairment group and no cognitive impairment group.Then according to the scores of HAMA and HAMD, the cognitive impairment group was subdivided into with or without emotional impairment two subgroups. At the same time, according to the scores of HAMA and HAMD, the same batch of patients were divided into emotional impairment group and no emotional impairment group. Collect the date of Fazekas, microalbuminuria, blood lipid, blood glucose, blood pressure, homocysteine, gender,age and other related clinical data, and then apply analysis of risk factors and multiple Logistic regression to find risk factors of cognitive and emotional impairments caused by CSVD.Results:1.Compared with no cognitive impairment group, according to analysis of risk factors, high systolic blood pressure(P=0.001), low education level(P=0.002), high diastolic blood pressure(P=0.032), low level of high density lipoprotein(P=0.039) had statistical difference(P<0.05) in cognitive impairment group caused by CSVD. Then according to the correction of multiple Logistic regression, high systolic blood pressure(P=0.002, OR=1.050), low education level(P=0.020,OR=0.587)had significant statistical difference(P<0.05).2.In the cognitive impairment group caused by CSVD,compared with no emotional impairment subgroup, according to analysis of risk factors, age, gender, education level, high systolic blood pressure, high diastolic blood pressure,diabetes, high cholesterol, high triglyceride, high level of low density lipoprotein, low level of high density lipoprotein,high uric acid, low level of folic acid, high homocysteine, high microalbuminuria had no statistical difference in the withemotional impairment subgroup(P>0.05).3.Compared with no emotional impairment group, according to analysis of risk factors, high microalbuminuria(P=0.049)had statistical difference in emotional impairment group caused by CSVD(P<0.05).4.Compared with no cognitive impairment group, the score of Fazekas of cognitive impairment group(P=0.053) had no statistical difference(P>0.05). Compared with no emotional impairment group, the score of Fazekas of emotional impairment group(P=0.609) had no statistical difference(P>0.05).Conclusion :1.High systolic blood pressure, low education level, high diastolic blood pressure, low level of high density lipoprotein have impacts on cognitive impairment caused by CSVD, high systolic blood pressure and low education level are independent risk factors for patients with cognitive impairment caused by CSVD.2.In the group of cognitive impairment caused by CSVD, age,gender, education level, high systolic blood pressure, high diastolic blood pressure, diabetes, high cholesterol, high triglyceride, high level of low density lipoprotein, low level of high density lipoprotein, high uric acid, low level of folicacid, high homocysteine, high microalbuminuria have no impact on with or without emotional impairment.3.High microalbuminuria has impact on emotional impairment caused by CSVD.4.Compared with no cognitive impairment group, the degree of white matter lesions of cognitive impairment group caused by CSVD has no difference. Compared with no emotional impairment group, the degree of white matter lesions of emotional impairment group caused by CSVD has no difference.
