Background:China is in the period of social transition and the medical market competition becomes more and more intense. High quality of medical service is the foundation of continuously and healthy development of medical institutions. Organization commitment is the key index of the team stability in a hospital and is also the important factor which impacts medical service quality. In the document for guidance of General Practitioners in July 2011, the State Council pointed out that, by 2020, the medical service model of "Primary treatment in primary medical units" should be established all over the country. Medical health industry is a special service industry. It is also a window of government service. These years, it becomes a hot topic of social attention and the focus of public supervision. The current situation includes lack of medical personnel, low social credibility, high intensity and risk, low payment, and lack of social support. A large portion of primary medical personnel are experiencing job burnout and their organization commitment is declining. Some even have demission intention. The investigation of organization commitment of primary medical personnel and the related factors in Hangzhou Economic and Technological Development Zone (HETDZ) may provide suggestions for raising the level of organization commitment and making the primary medical team stable.Aims:Based on the survey of organization commitment, social stress, social support level, Effect-Reward imbalance, simple coping style and their influences on organization commitment, this study tried to provide the evidence of how to improve the organization commitment of primary medical personnel in HETDZ for Health Administrative Departments and human resource department in colleges and universities.Methods:250 primary medical staffs from 8 colleges and 2 community health service centers in HETDZ participated in the survey. Demographic information, employee organizational commitment scale, Effect-Reward imbalance scale, Copenhagen psychosocial questionnaire were used in the survey. Descriptive analysis, t-test, one-way analysis of variance, spearman correlation and multivariate linear regression were used in data analysis. The current study explored the level of primary medical staff organization commitment and the related factors.Results:(1) 250 individuals were investigated in this survey.243 questionnaire were returned, and 224 were valid (92.18%). The subjects aged from 22 to 77 yo, and the average is 38.20±10.91 yo. (2) The total score of organization commitment is 61.86±11.95. The subscales ranged from high to low are:standard commitment, affact commitment, ideal commitment, opportunity commitment, and finance commitment. It also shows from the influences of demographic characteristics to organization commitment that the organization commitment is statisticly different in primary medical personnel with different agencies, genders, hospital departments, medial staff types, professional titles, employee attributes, overtime work, income level and time on the way to work. (3) The dimensions of the social psychological stress level ranged from high to low are:interpersonal relationship, leadership, job satisfaction, individual development, job requirement, and job instability;(4)The total score of social support is 43.05±7.37, among which objective support score is 10.28±3.18 and the subjective score is 24.71±4.73. (5) The score of payment is 14.43±5.16, the score of reward is 25.91±7.33, and ERI ratio is 1.07±0.42; (6) The positive response score is 35.69±5.58 and the negative response score is 17.78±4.07; (7) Individual development has significant influence on affect commitment and ideal commitment (p<0.05); positive response has significant influence on affect commitment, standard commitment and ideal commitment (p<0.05); job requirement has significant influence on affect commitment, standard commitment and ideal commitment (p<0.05); negative response has significant influence on finance commitment and opportunity commitment (p<0.05); reward has significant influence on opportunity commitment (p<0.05); the individual development of social psychological stress level, two dimensions of job requirement, the positive and negative response of simple coping styles have significant influence on the organization commitment of primary medical personnel (p <0.05)Conclusions:(1) the organization commitment of primary medical personnel is relative low. The demographic characteristics such as gender, hospital department, income, professional title and employee attribute are influence factors. (2) the social psychological stress level of primary medical personnel is high and social support is relative low.(3)The level of Effect-Reward imbalance is high payment and low reward. The dimension of positive response is higher than negative response, which is among the national average level. (4)The social psychological stress level and simple coping styles, especially the negative response of simple coping styles and job requirement of social psychological stress level, have a highly predictive effect on organization commitment.
