Objective:Research different operative methods for rhegmatogenous retinal detachment after retinal detachment repairs the influence of macular area, to observe the incidence of subretinal effusion, analysis the influence factors of prognosis of visual acuity.Methods:The involvement of macular hole retinal detachment in 32 patients divided into 34 groups according to surgical procedures, including SB 20 scleral surgery group( including scleral buckling or scleral buckling), PPV group 14 vitrectomy joint eyes filled with inert gas technique, postoperative ophthalmic OCT detailed examination and inspection, the average follow-up of 3-12 months, the incidence of postoperative retinal relatively effusion,macular micro- structural changes and postoperative visual prognosis, statistical analysis of parallel processing.Results:Retinal fluid, 34 eyes of 14 eyes(41.18%), SB group of 12 eyes, accounted for 60%;PPV group 2 eyes, accounting for 14.29%. Incidence of PSF compared two groups, the difference was statistically significant(P = 0.013 < 0.05), namely SB group the incidence of subretinal effusion is higher than the PPV. 7 eyes IS uneven/OS layer reflection, missing,accounted for 20.59%, SB group 4 eyes, accounted for 20%; PPV group 3 eyes(21.43%).Is similar between the two groups have no statistical significance(P = 1.000 > 0.05). 9eyes obviously thinning, center concave neurological cortex(26.47%), 5 eyes SB group,accounting for 25%; PPV group 4 eyes, accounting for 28.57%. Is similar between the two groups have no statistical significance(P = 1.000 > 0.05). Macular pucker, 4 eyes(11.76%), SB group 3 eyes, 15%; PPV group 1 eye, accounted for 7.14%. SB group 1 eye macular edema, macular hole 1 eye, have accounted for 2.94%. SB group vision after surgery: 0.23±0.13, PPV groups: 018± 0.14, two sets of vision recovery is no statistical difference(t = 1.141, P = 1.141 > 0.05).Conclusions:(1) different operative methods for rhegmatogenous retinal detachment after retinal detachment in the macular area there are differences in the impact of subretinal effusion,and the incidence of PSF SB group was obviously higher than that of PPV group.(2) the PSF, absence of photoreceptor cell layer, or central sunken neurological thinning of the cortex may be the main factors influencing the postoperative visual acuity recovery.(3) the sclera retinal detachment surgery is surgery or minimally invasive vitrectomy, still needs to be made in large sample studies and long-term follow-up.
