Reconstruction Of Medial Patellofemoral Ligament(MPFL)for The Treatment Of Recurrent Patellar Dislocation Assisted By Arthroscope.

Objective To evaluate the clinical effectiveness of medial patellofemoral ligament(MPFL) reconstruction for the treatment of recurrent patellar dislocation assisted by arthroscope.Methods 35 cases selected from october 2012 to october 2014 in our hospital,all patients including 10 males and 25 females with recurrent patellofemoral dislocation received the MPFL reconstruction assisted by arthroscope.The average age was 19.3years(ranged from 15 to 40).The average time from the last patellar dislocation to surgery was 5months(ranged from 1day to 10 months).At the same time, exploring all around inside the joints under the arthroscopy,15 cases with the free cartilage and osteochondral fragment taken out,20 cases with lateral patellar retinaculum released.Results 33 cases with the entire follow-up from 15 to 20 months,he average follow up duration was 18 months,2 cases were lost.No recurrent episodes of dislocation or subluxation occurred.All the patients showed negative apprehension test at 0°nd 3°lexions of knee which can be fully engaged in full sports activities,The range of motion of knee restored normal 1 year after operation.The Lysholm score was improved from 70.57 ± 2.97 preoperatively to 92.48±3.07 postoperatively and Kujala score was improved from 71.78±2.32 preoperatively to 92.82±2.14 postoperatively,both showing significant differences(P < 0.05).Conclusion MPFL reconstruction for the treatment of recurrent patellar dislocation can be significantly improved its efficacy reliable and satisfactory,Arthroscopic techniques is minimally invasion,fewer complications, faster recovery and good effect. MPFL reconstruction is a challenging surgical procedure that requires experience to avoid complications related to malpositioning of the femoral tunnel or inadequate tension on the graft.we can find potentional patellar instability factors and observe the motion of patellofemoral joint dynamically by help of arthroscope,it will provide help to success of surgery.
