Object:To detect the change of plasma gelsolin level in normal volunteers, type 2 diabetes patients, cerebral infarction patients,type 2 diabetes combined with cerebral infarction and explore its function on neurological damage and prognosis evaluation in patients with acute cerebral infarction type 2 diabetes combined with cerebral infarction.Methods:Plasma gelsolin level was detected using enzyme-linked immunosorbent assay(ELSA) in a total of 120 cases of observation objects, divided into 4 groups: healthy control group, type 2 diabetes, cerebral infarction group, type 2 diabetes patients with cerebral infarction, 30 cases in each group(The control group, only measured once plasma gelsolin levels, measuring the incidence of cerebral infarction within 48 hours and 14 days after onset of plasma gelsolin levels.).①Compare the plasma gelsolin levels between 4 groups.②The cerebral infarction patients were grouped according NHISS in the cerebral infarction : compared with scores at admission decreased scores ≥4 points or 14 days after onset NHISS score of 0 points were good outcome group, and compare scores at admission decreased <4 into poor prognosis group. Compare plasma gelsolin levels of two groups and prognosis in patients with cerebral infarction. ③The modified Rankin Scale score( mRS)was conducted 3 months after the onset, and its correlation was analyzed with plasma gelsolin levels in patients at admission, 14 days after onset of cerebral infarction.Results:①Plasma gelsolin levels was significantly different in plasma gelsolin levels at admission of four groups(p <0.01).The gelsolin levels in healthy control group is higher than the other three groups,(126.11 ± 38.03) ng · m L-1 vs(69.61 ± 23.20) ng · m L-1,(77.60± 14.78) ng · m L-1,(63.41 ± 21.17) ng · m L-1(p <0.01).Plasma gelsolin levels in Cerebral infarction in diabetic is below than infarction group(p <0.05); the plasma gelsolin levels between in diabetic patients with cerebral infarction group, cerebral infarction group and diabetes group was not statistically significant(p >0.05).the plasma gelsolin levels of all patients with cerebral infarction at admission were significantly higher than at 14 days after onset(p <0.01).The plasma gelsolin levels of cerebral infarction with diabetes group were lower than the cerebral infarction group at the14 days(P < 0.05);②Compared with the cerebral infarction with diabetes group,the cerebral infarction group have a good outcome(P< 0.05);The plasma gelsolin levels in Good prognosis group is higher than poor prognosis group(P <0.05).③The correlation analysis showed that the plasma gesolin level was negatively correlated with m R S scores in cerebral infarction patients. The Spearman correlation analysis showed that the plasma gesolin level was negatively correlated with mR S scores in cerebral infarction patients not only at the time of admission but also at the first 14 days of the onset(r =-0.706,-0.631, all P <0.01),and also in cerebral infarction with diabetes group(r=-0.672,-0.725, all P <0.01).Conclusion:1.Cerebral infarction patients plasma gelsolin levels were lower than normal people, type 2diabetes patients with cerebral infarction reduce more.2.T2 DM patients with cerebral infarction plasma gelsolin levels were negatively correlated with the prognosis of patients.Plasma gelsolin levels have a certain clinical value in the assessment of the disease and prognosis of T2 DM combined cerebral infarction.
