Objective To investigate the effect of D-dimer and B type natriuretic peptide(BNP)on the prognosis of acute cerebral infarction.Methods A retrospective analysis was performed on 124 patients with acute cerebral infarction who were admitted to the hospital from January to December in 2017 in Caoxian People's Hospital as subjects of the observation group.Another 123 patients with non-cerebrovascular disease at the same time in this hospital were selected as control subjects.Serum NT-proBNP levels,D-dimer concentrations,NIHSS scores,and cerebralinfarction size were measured in two groups of patients.Changes in D-dimer concentrations and NT-proBNP levels in patients with acute cerebral infarction were analyzed.Correlations between cerebral infarct size and D-dimer concentration,blood NT-proBNP levels,and NIHSS scores were analyzed.The relationship between the severity of the disease or hypertension and D-dimer concentration,blood NT-proBNP levels was analyzed.And the impact of D-dimer concentration and blood NT-proBNP levels on the prognosis of cerebral infarction was also analyzed in the study.Results(1)Comparison of D-dimer concentration and serum NT-proBNP levels between the patients with acute cerebral infarction patients and non cerebrovascular disease: The two indexes of patients with acute cerebral infarction were higher than those of patients without cerebrovascular disease,and the difference was statistically significant(P<0.05);(2)The effects of different disease severity on D-dimer concentration and blood NT-proBNP levels in patients with acute cerebral infarction: According to the GCS Coma Scale,the two indicators in mild-moderate patients were significantly lower than those in severe patients,and the differences were statistically significant(P<0.05).According to the NIHSS score,the two indicators of mild to moderate patients were significantly lower than those of severe patients,and the difference was significant(P<0.05);(3)The effect of combined hypertension on D-dimer concentration and blood NT-proBNP levels in patients with acute cerebral infarction: D-dimer concentration and blood NT-proBNP levels were significantly higher in patients with combined hypertension than in normotensive patients,and the difference was statistically significant(P < 0.05);(4)The effect of cerebral infarct size on D-dimer concentration and NT-proBNP levels in patients: D-dimer concentration and blood NT-proBNP levels were significantly higher in patients with large-area cerebral infarction compared with non-macroinfarcted patients,and the difference was statistically significant(P < 0.05);(5)Effects of D-dimer Concentration and Blood NT-proBNP Levels on the prognosis of patients with acute cerebral infarction: Comparing D-Dimer concentration and blood NT-proBNP levels in death and survival patients,the two indicators of the death group were far higher than those of the survival group,indicating that the high levels of the two indicators may affect the patient's mortality rate,and the low level of indicators may promote the improvement of the disease.The difference between the death group and the survival group was statistically significant(P<0.05).Conclusion Acute cerebral infarction will lead to increased D-dimer concentration and blood NT-proBNP levels.In patients with acute cerebral infarction,different disease levels,combined hypertension,and cerebral infarction area will all affect D-dimer concentration and blood NT-proBNP levels,and D-dimer levels and blood NT-proBNP levels affect the prognosis of patients with acute cerebral infarction.The higher the level of plasma NT-proBNP and D-dimer in patients with acute cerebral infarction is,the more severe the condition is,and the worse the prognosis and the higher the mortality are.Therefore,D-dimer concentration and plasma NT-proBNP levels can be used as important markers for the diagnosis,observation,treatment and assessment of prognosis of acute cerebral infarction.
