0bjective To observe the plasma levels changes of ADMA (asymmetric dimethylarginine, ADMA) in acute cerebral infarction among the type 2 diabetics (non- insulin dependent diabetes mellitus, NIDDM) and its correlative factors, its relevance to the pathogenesis of acute cerebral infarction.Methods Study observed total 157 cases that included 64 cases of type II diabetics (including 33 cases with acute cerebral infarctions event and 31 cases without any Acute cerebral infarction events ), 59 cases non-diabetic acute cerebral infarctions, and 34 cases of normal contrast group. All the cases were subjected to monitoring of biochemical parameters such as ADMA, SDMA (asymmetric dimethylarginine, ADMA), CRP,lipids and Uric acid etc, with high-performance liquid chromatography-tandem mass spectrometry. Collected data were subjected to comparative analysis, analysis of correlation and logistic analysis to identify the risk factors for the acute cerebral infarction.Results The plasma levels of ADMA in the group of type II diabetics with acute cerebral infarction(0.75±0.11mmol/L) increased significantly in comparison to normal control group (0.53±0.14mmol/L P<0.05), and compared to the group of type II diabetics without Acute cerebral infarction(0.61±0.87mmol/L), as well (P<0.05).The levels of ADMA among the non-diabetics with acute cerebral infarction(0.59±0.14mmol/L) were also significantly increased compared to the normal control group (P<0.05) . There is no clear correlation between ADMA and the common risk factors for ischemic cerebrovascular disease, such as age, blood pressure, total cholesterol, blood sugar, etc. but it is positively correlated with low-density lipoprotein (LDL)(r=0.232 P<0.01), SDMA (r=0.287 P<0.01)and ADMA/SDMA(r=0.383 P<0.01). Logistic regression found that risk factors such as plasma ADMA concentration, gender, weight, systolic blood pressure, hypertension, history of atrial fibrillation, and low density lipoprotein are having close relevance to acute cerebral infarction. Conclusion Plasma levels of ADMA is significantly elevated in patients with acute cerebral infarction,more significantly among those with type 2 diabetes, indicating that ADMA plays a key role in the occurrence and progression of the acute cerebral infarctions associated with type II diabetes. Hence, ADMA can be considered as one of the important pathophysiological basis of acute cerebral infarction in type II diabetics and at the same time, ADMA may also act as an independent risk factor for acute cerebral infarction. ADMA may be a new drug target for the prevention and therapy of acute ischemic cerebrovascular disease.
